Why is the tip of my nose is deformed, and what can I do about it?
Nasal and septal deviation
Aug 17, 2020
Hello,
You are absolutely correct in your observations. The end of your septal cartilage is deviated to the right side displacing the columella of the tip to the left side and making it appear asymmetric. This type of septal deviation may also lead to difficulty breathing through the nose.
Your nasal dorsum is deviated to the right side and appears wide on frontal view. The nasal tip is slightly bulbous and overprojected (sticks out too far). In order to properly correct all of the above,…
Considering a nonsurgical nose job before a revision rhinoplasty. Will I be able to dissolve th
Non-surgical rhinoplasty before revision surgery
Aug 4, 2020
In my opinion, non-surgical rhinoplasty is not a good test run for revision rhinoplasty as these are completely different procedures and the results that can be achieved with one versus ano her are completely different. If you are not planning to have revision surgery any time soon then getting a temporary improvement with a filler is a good option. However, I would not do non-surgical rhinoplasty if you are planning a revision surgery within next 6 months or sooner. The filler may obscure…
Which type of filler and amount of should I use for my tear trough lines?
Tear trough fillers
May 25, 2020
Most commonly used fillers for tear trough areas are Restylane Silk or Belotero, which are thin hyaluronic acid based fillers that don’t attract much water do not to cause a lot of swelling under the eyes.   Looking at your photo, you should probably start with 1 syringe (half per side, or maybe even less)- it is always better to be more conservative with tear trough areas.  Nasolabial folds usually require “thicker“ fillers which can give patients more augmentation and more filler longe…
For eyeglass wearers, can a rhinoplasty change the position of where your eyeglasses sit on you
Rhinoplasty and glasses
May 25, 2020
You don’t want to change your nose to fit the glasses. On the contrary, the glasses are usually adjusted to fit the nose. Having said that, dorsal augmentation can absolutely be a part of rhinoplasty surgery if the main goal is cosmetic improvement of the nose. If you think that your bridge is too low and would like to make it higher for cosmetic reasons and not for the reason because opticians told you that it would fit your glasses better, then you should consider it.
For rhinoplasty, can we test the final look with filler and have them removed immediately for s
Surgical versus non-surgical rhinoplasty
May 21, 2020
Hello. I suggest that if you are really thinking about surgery and long term correction, then don’t do the filler before. The filler obscures the surgical process, it is not always possible to fully remove it as some of the material incorporates into the soft tissues. Plus there is a cost involved with trying a filler… Rather consider doing simulation photos with your selected rhinoplasty surgeon to see to proposed result. However, if the surgery plans are not in the immediate futur…

Dorsal augmentation and brow movement
May 15, 2020
Hello. While dorsal augmentation may affect the appearance of the skin area between the brows, it should not affect brow movement

Rhinoplasty to increase nasal dorsal height
May 15, 2020
There are several surgical options for dorsal augmentation (increasing the height of the dorsum): 1) cartilage graft (from the septum or from the rib if large amount of augmentation is needed); 2) diced cartilage wrapped in fascia or Alloderm; 3) nasal implant (Gortex or silicon based). The need for osteotomies depends on the physical exam and the width of the bony dorsum.  It is not always that the osteotomies are required as dorsal augmentation by itself effectively creates an ap…
Why is my nose uneven and why is it black? What can I do?
Nasal pigmented lesion
May 15, 2020
It seems that you have an area of melasma or a pigmented nevus on the columella (bottom part of the tip). Your nose also looks widened with horizontally oriented nostrils.   you may consider rhinoplasty surgery to improve the shape of the nose overall and to remove pigmented area on the columella.

Teenage rhinoplasty
May 11, 2020
Hello. Fully understand your concerns about having a surgery. Teenage rhinoplasty has become a very popular procedure over the last decade, with many high school students aiming to improve nasal appearance and/or breathing before starting college. It is widely accepted that rhinoplasty can be safely performed after 16 years of age, when the nose stopped growing and the facial features are fully formed. The surgery is performed under general anesthesia.   The recovery is actually not a ver…

Rhinoplasty for dorsal hump and tip deprojection
May 9, 2020
Hello. Based on your photos it seems that you would benefit from dorsal hump reduction and slight tip deprojection.   Narrowing of the nasal bones is also performed almost always as part of dorsal hump reduction. These are the very common components of most rhinoplasty surgeries. The cost of the surgery would largely depend on the surgeon you decide to go with and the geographic area, and not so much on the components of the surgery.
What are my options for earlobes that stick up and outward?
Ear lobe correction
May 9, 2020
There are several options to correct protruding ear lobes.   First option involves removal of a triangular portion of the earlobe skin from the back surface.   When resultant area is close with sutures the earlobe gets pulled back.  Second option involves suturing earlobe to the opposing area of the face thus essentially making the free hanging portion of the earlobe shorter. All of the above options are done in the office under local anesthesia and the procedure should take under an hour f…
Will moving the upper jaw only downward affect the nose in jaw surgery?
Is rhinoplasty affected by jaw surgery
May 7, 2020
If you are planning  an orthognatic surgery, it is better to be done before rhinoplasty, if possible.   Any surgical manipulation of the upper jaw (maxilla) can affect rhinoplasty result as maxilla serves as the foundation of the nose.
30 F: how to treat sagging corners of mouth and somewhat botched filler?
Droopy corners of the lips, masseter hypertrophy
May 7, 2020
Hello.   Based on your photos, your masseter hypertrophy has recurred and this is what gives your face a squared angulated look. Would definitely consider repeating botulinum toxin treatment there which should help the problem, especially if you repeat treatment regularly (every 3-4 months) and avoid gum chewing.  I would also try using a small amount of BT to the corners of the lips trying to lift those up.   As far as lower cheek irregularity is concerned, this could be related to prior s…
Is it possible to achieve my dream nose (lower the radix and dorsum)?
Hump reduction and tip deprojection rhinoplasty
May 3, 2020
Hello.   it is possible to reduce the dorsal height and deproject nasal tip to achieve the changes you desire. Based on the frontal view, your tip would also benefit from refinement to match the smaller nose.   The reduction of the nasal size should not be done very aggressively as it may lead to breathing problems.

Problems breathing through the nose after trauma
May 3, 2020
Hello.   it is possible that your breathing issues are due to the deviated nasal septum, which could have resulted secondary to prior nasal trauma. Other possibilities are sinusitis or allergic rhinitis, both of which cause swelling inside the nose and nasal obstruction.   You need an intranasal examination to determine what the cause of your breathing problem is.  Deviated septum will require a surgical correction (which may be combined with rhinoplasty if you desire any cosmetic improv…
