Fat cells from adults may increase or decrease in size based on caloric intake /metabolism  If you lose weight following fat grafting you may lose volume.  The loss of volume following fat transfer is more a function of cell loss since the take or survival of those cells is not 100%. The biology that produced loss of facial fat and volume continues abated.  In other words repeat fat grafting is very common.  Regards.

The reason your jowls are worse post-op is probably due to swelling.  Facelifts  can alter lymphatic drainage of the face and may take up to 6 months to normalize.  This means that they will improve with time.  Some surgeons can improve jowls with a lateral smasectomy,  but for me the  best results are achieved with a  supra-zygomatic SMAS dissection and  a strong vertical vector. The occasions are rare when I do not do a playsmaplasty/neck lift as part of a SMAS facelift.  I am also oftentimes surprised at the amount of redundant skin that is apparent,  following undermining , in necks that otherwise appear youthful.  Based on your photo I would include neck lift  as part of your facial rejuvenation.

I would suggest an ultrasound to ascertain whether you have fluid in the capsule.  You may have torn the capsular tissue which can often times bleed which in turn causes inflammation.  Regards.

The length of the incision is based on the redundancy of the skin.  The greater the skin the longer the scar.  You can limit the length by leaving dog ears, not desirable or adding a vertical component, even worse. Regards.Dr Kiener

Restoration or correction of facial volume loss can be accomplished with filler, fat or implants.  There are pros and cons to each technique. Based on your photos you have a prominent mid-facial groove which can be caused by a relative lack of  mid facial fat.  My preference is to treat this area with fat grafting although fillers can be effective as well.  Regards.Dr Kiener

Depending on your age and quality of tissue your breasts may return to a pre-operative appearance following explantation.  It may take weeks to months for that to occur. Personally,  I always perform a platysmaplasty as part of a  formal SMAS facelift. However different results can be realized by different surgeons performing the same technique and conversely similar results can be seen with different techniques.  Based on your photos I would perform a  platysmaplasty utilizing a sub-mental incision transecting ,i.e.cutting,  the platysma muscles and suturing them together in the midline.   I would also address the platysma posteriorly as well as a  suprazy...
