Excellent question - and there are many correct answers. Many facial plastic surgeons perform simultaneous fat grafting in the cheeks and under eye area to improve volume deficient regions that are not commonly improved with the facelift alone. You’re correct in that 30-70% of the fat will be resorbed as you heal. This may mean that an additional volumizing agent may be required in the future - but when done properly in experienced hands it provides a great result.

Great question - controlling the precise rotation of the tip after rhinoplasty is essential to getting a great result. No matter the technique, once the surgery is complete gravity has an effect and the rotation descends to some degree. Depending on the approach this may be a barely discernible amount to a large amount. Because of this, the nasal tip is often rotated higher at the conclusion of the procedure so that it will settle in the first 6 months to the ideal position.

The long recovery after an open rhinoplasty can be frustrating. The cast placed during the first week keeps the swelling down and the cartilage in place but once removed the nose expands. Thanks for your question. Repairing your deviated septum, asymmetric dorsum and any other changes you’re hoping for does not require that your nose be increased in width. The changes you’ve mentioned are able to be achieved through both an open approach (with a small external incision) and a closed, endonasal approach (only incisions within the nostrils). Thanks for your question. I agree that often hooding of the eyelids can be caused both by natural aging - with descent of the brows and thinning of the skin - and hereditary. Blepharoplasty is often performed in younger patients for reasons just like you’ve mentioned in your question. A conservative, skin-only, blepharoplasty performed in clinic would be very helpful to achieve the appearance you’re hoping for.
