You would benefit from a breast lift procedure. If you are bothered by the bra bulge, performing liposuction in this area is a nice way to contour the outside border of the breast. If you would like to increase your breast volume as well, this can be done without implants using a fat transfer procedure.
Do you believe this would be an augmentation or a mastopexy also?

The best way to know for sure is with an in-person consultation, but judging by your photos I would recommend an augmentation only. If you are dissatisfied with nipple position, you can consider adding a mastopexy at a second stage (3-6 months after your initial surgery).

You don’t need a lift in the traditional sense, but in tuberous breast cases, the breast fold needs to be lowered and a suturing technique is used to correct the shape of the areola. This places a scar circumferentially around the areola like a circumareolar lift does.

Both your breasts are constricted. The smaller breast is tuberous and the larger breast has some constriction near the center of your chest. To correct this, you will likely need some combination of asymmetrically sized implants and breast lift/breast reduction procedures. Because of your asymmetry, you should consider performing this in a staged fashion. In severe cases of breast constriction, insurance will cover surgery.
Are there any solutions for outward facing nipples/breasts?

If your nipples are facing outwardly, unfortunately breast augmentation will not correct this. There is a possibility that this might be exaggerated by a breast augmentation. A procedure known as a circumareolar mastopexy can be added to medialize the nipples (move them in).

You would benefit from a brow lift, high SMAS face and neck lift, upper eyelid lift, and fat transfer to the face. Fillers and Botox will only create minor changes, and if you are interested in a more long lasting result, I would recommend surgical intervention.

Cup size can be deceiving in breast augmentation, because it varies so much from one bra manufacturer to another. For example, a Victoria’s Secret D cup is very different from other manufacturers’ D cups. As a general rule of thumb, every 150-200 CCs is about one cup size, so you will likely be 1.5 – 2 cup sizes larger.

You might benefit from a breast lift procedure. These procedures can reshape the breast and move the nipple to a perkier position. Asymmetries can also be addressed with this procedure.

Unfortunately, fillers will provide minimal benefit for crepey skin around the eyes. Adding volume with a filler to this area can give you a mild improvement, but you would be best served with a lower lid blepharoplasty +/- fat transfer to the tear trough and midface. This would give you a more definitive treatment (and probably save you money in the long run).
If you really want to hold off on lower eyelid surgery, another option you could consider is a very precise, low-dose Botox…
I have very pronounced collar bones. Can filler be used to fill the hollows in this area?

It would likely take a very large amount of filler to fill in the shadows in this area. I would advise caution with filler in this area as well, because the skin over the neck is very thin, and there is some very important anatomy in this area.
