I don't think that buccal fat removal will make jowls worse in the future. Jowls will form based on your genetics, lifestyle, and skin care. Buccal fat removal will make you look more angular, and the volume can easily be replaced with fat or a filler if necessary at any time. Chin implants, when necessary, are great at improving the jawline! Both can be done at the same time right in the office.

You can see if it's the gland by injecting the area with Botulinum (eg. Botox). This will shrink the gland and get rid of that mass. It could even be a permanent fix in some cases.

When we see a horizontal line like that it often indicates that the lip is too long and struggling to lift. A lip lift ( a procedure I largely reintroduced ) can be done with a rhinoplasty to correct both issues.
