16 months post-op & breathing became pretty bad a few months ago, what is inside my nose?
Breathing issues post rhinoplasty
Dec 5, 2023
Hello, Although it is hard to say from a photo, based on your description and what can be seen, it is possible that the “lateral crura” part of the lower cartilages is recurring back into the nose. This is a form of nasal valve obstruction. Turbinates and other types of obstruction require an in-person exam to evaluate. For now, breath right strips can help during exercise and sleep, but make sure to bring this up to your surgeon, or visit with another qualified rhinoplasty surgeon. Good luck.
What type of rhinoplasty to fix how the end twists making my nostrils uneven?
Uneven nostrils
Dec 4, 2023
Hello,In this case the nostril asymmetry is mostly due to the tip of the nose leaning to side. The center part is the columella, it’s direction is dependent on the bottom part of the septum and the lower lateral cartilages.Imagine the nose as a tent from that 4th bottom view picture. The tent has two side walls and a center post (columella). If you lean the top of the tent to one side, the opening will become uneven in the same way. This wouldn’t be a specific “type” of rhinoplasty … the…
Which procedure would target excess skin in the outer corner of the brow while also restoring almo
Upper lid and brow hooding
Nov 30, 2023
Hello, if you had filler in the upper eyelids, this can be a possible reason. Filler for the upper eyelids can be nice to treat a ball in socket type appearance, which can be the result of over aggressive surgery but can occur naturally as well. It’s something I find applicable on a very rare basis. Even Filler from 4 years can stick around and create fullness. I would first try to dissolve this area. If not, a lateral brow lift and brow fat pad pexy can address that lateral holding. When…
Under eye wrinkles/folding skin – what to do?
Lower eyelid folds
Nov 30, 2023
Hello. This is actually a very common issue. I have seen patients ask to treat this with botox, but unless you paralyze the zygomaticus muscle (smiling muscle) it will not help. The reason for the issues is two fold. There is some skin laxity in the area and the skin itself is very thin, with decreased amounts of collagen. When we smile, the cheek raises up and the skin wrinkles as it bunches up. The more conservative treatments would be things like lasers (CO2, fraxel), microneedling (with…
Will a tip rhinoplasty (only) help my issue?
Tip revision rhinoplasty
Nov 30, 2023
Hello. Yes it is possible to address the tip only. Every (good) rhinoplasty is very specific to the person’s desires, nasal anatomy, and the facial aesthetics. Issues occur when noses are treated with the same “generic” techniques across the board. From the pictures it looks like the main issue is the tip cartialge orientation, specifically that they are cephalically mispositioned and bowed down below the tip. I see these in revision cases very frequently and they are often due to over…
Closed Rhinoplasty to deproject/ reduce length of a long ethnic nose?
Open vs closed rhinoplasty.
Nov 29, 2023
The open vs closed rhinoplasty has been going on for decades. Closed was out of favor for a long time but has remerged in the recent decade as it sounds more appealing to patients. Without getting into the controversial points, I personally prefer open. The incision heals well, my patients recover quickly, and I get the precision that I prefer in each case. I also find that closed requires more incisions inside the nose and more stretching / pulling on the skin. One other thing to consider…
Is this due to a deviated septum or something with the nasal tip?
nasal tip
Nov 27, 2023
Hello, It appears you are referring to the fullness created by the lower lateral cartilages, specifically the lateral crural portion. These cartilages give shape to the tip and lower part of the nose. They can easily be manipulated but also have memory and a “bounce” effect as your are describing. To be sure you should consult with a qualified facial surgeon. Hope that helps!
