Having lost 90 pounds, I would predict that you, will likely need plication of the rectus muscles to get a flat stomach whether you have a divarication or not!It is in the rare patient, with perhaps washboard abs, no fat and severe stretch marks, that this would not be carried out. I would go as far as to say that I would not plicate such a patient even if she had a divarication.

Since your capsules were loose and had to be tightened, it is common sense that maneuvers to stretch them such as massage are contraindicated for now. However, if you were to develop some firmness indicating a capsular contracture, then I am sure your surgeon would want you to start massaging them again.

Yes, you have some necrosis of the nipple-areolar complex. In the case of a breast reduction, this is a well recognized complication and the recommended treatment is to manage it conservatively with dressings, as you are doing. You would expect it to heal with a slightly widened scar, which could be revised later. The situation is more complicated if an implant is involved. There are only two effective methods of removing hyper-dynamic neck bands in a thin neck. One is by repeated Botox injections and the other is by complete division of the platysma, in your case at the same time as facelift.

Congratulations on your new baby! Please do not consider breast surgery until at least 3-6 months after you stop breast feeding! Having said that you certainly do have grade 2-3 ptosis (droopiness) and this is unlikely to resolve spontaneously in my opinion. The good news is: you do not have tubular breasts.
When things settle down you may consider an augmentation, a breast lift or both. The size of your nipple-areolar complex can be adjusted at that time if it is still a problem.

I am sorry you are not happy with your result from breast augmentation. There are a number of issues here. The first is size. Clearly the size is not what you expected. OK, I am biased. I am a full professor at UCLA. The Plastic Surgery Resident Teaching Clinic is staffed by residents and closely supervised by faculty surgeons such as myself. The fees are much less than in private practice and a special low rate has been negotiated with the hospital’s top notch day surgery facility as well as with Anesthesia.
