I generally think having one anesthetic and one recovery is best. However, there are limits to a surgeons ability to operate skillfully over very long cases. There is no other procedure to show more teeth without an external incision. You’re correct, a V-Y plasty would not increase dental show!
If done well, the external incision should never be visible to others. It can be further hidden with lasers postoperatively

You are an excellent candidate for cheek augmentation. The volume loss we experience is due to bone loss and fat loss. Fat can be liposuctioned from your abdomen or other areas of the body and can be injected to more permanently augment the cheeks whereas filler provides temporary relief but allows you to see what a permanent result would be like. It will fade more with time. An IPL laser would help resolve it faster and you would likely only require one or two treatments. I would highly recommend diligent sun protection to prevent further pigmentation. Some people have success lightening the discolored skin with the use of prescription creams that contain hydroquinone, these require a prescription in some states.
