You are very observant. This is a syndrom that I have described in the literature. We called it Oculonasal synkenesis. A branch of the nerve that supposed to go to the eyelid muscle can misconnect with a muscle in the nose. You can either have this muscle paralized with Botox or be removed surgically. It usually does not create this size bulk. Regardless, it can be fixed.

Any type of facial asymmetry can be corrected. A facelift does not seem to be a good choice for you. Ideally you should have your upper and lower jaws rotated around. However, if that is too complicate for you. Fat injection and moving your chin forward and left may help along with the tooth extraction. You may want to consult a plastic surgeon with craniofacial surgery training.

Yes, your nose can be made more feminine. Your nose is too wide in the bridge, it is deviated to the right, your tip is too wide and asymmetric and your nostrils are slightly asymmetric. On the profile the nose is over-projected and your columella (the column between the nostrils) is retracted. Additionally, your headaches, if they are sinus related can be improved or eliminated. A protruding chin can either be set back by cutting the the chin bone horizontally through an incision inside the the lip or by shaving the bone down using a small incision under the chin. You may consider consulting a plastic surgeon who has experience doing this surgery.

Your nose can be improved a good deal since it has a number of flaws. If you are comfortable with your previous surgeon it would be less costly to go back to your surgeon. Otherwise you should choose someone who has a great deal of experience in revision surgery since this is not going require enough knowledge of secondary rhinoplasy.

Any asymmetry in the nostrils can be improved and the nostril flaring can be corrected. You have wide nostrils and the right nostril is higher than the left side. Additionally your tip is uneven, more projected the right, the tip is tilting to this side and it could have more pleasing definition. There is some fullness above the tip on the right as well. These all can be eliminated or improved to a great deal.

By lifting the columella using a graft, like putting an additional post under a sagging tent, the tip can be lifted, the nostrils will narrow and the columella can be retracted at the same time. The nostrils can be further narrowed if needed.

No, there is no reason for the tip rhinoplasty to be more expensive than a full rhinoplasty. Essentially every rhinoplasty includes the tip rhinoplasty and more and it does not make sense to charge more for the tip rhinoplasty.
