Thank you for your question and photos. It’s hard to say whether the cause is acromegaly or prominent brow bone from other genetic factors. The treatment is the same which is a brow bone contouring procedure. You will likely need a type 3 cranioplasty to contour the forehead. I do this weekly so make sure you consult a surgeon who does it very often. Incisions are hidden in the hair and recovery in my patients is 1-2 weeks.

I think a rhinoplasty to straighten and refine your nose can make a big difference for you. You do have some deviation of the bone so they need to be moved slightly to make them straight. Breaking the nasal bones is not a major procedure and I don’t think you should be hesitant about it if you’re going to have a rhinoplasty. Looking at these photos, I think you would benefit from a lip lift to improve the upper lip proportion. It can give you more red lip show, more teeth show, and it can improve the smile. Keep in mind that smiling is a dynamic action of muscles so some aspects of it cannot be changed but overall you would most likely be happy. I don’t think Botox to the masseter muscles will give you the change you’re looking for unfortunately. I see what you mean in this photo. I performed large number of forehead reduction and as long as the new hairline has a natural shape and placed in the appropriate position (not too low, not too high) then your face can look less elongated but not look like it just stop suddenly with a straight line. For a good and experienced rhinoplasty surgeons, we do it all the time. It is much easier to lift up (rotate) the tip than to drop it (de-rotate it). For patients have a shorter nose (you may have a shorter nose based on the photo) then moving the tip down is a good idea and I’m glad you can see and recognize that. It helps when the patient has a good eye and understands these aesthetic points. Best wishes.

I understand your tip issues but I think the bridge is not quite smooth as there is a tiny depression between the upper bony part and skin of the tip and that needs to be addressed along with the tip. It doesnt add much to your experience compared to just a tip plasty. I recommend you try to get everything to be balanced and be done. So you don’t have to worry about your nose again. Best wishes
