From your pictures, your nasal bones do not appear to be wide. A smooth transition from brows to nasal tip are recommended. As a result, a more balanced appearance would be achieved by narrowing and creating a more harmonious tip. Only narrowing the dorsum would accentuate the width of your tip and not be desirable in my opinion.

It is difficult to assess from your photographs. An in person evaluation would likely be most beneficial in your case. In the photo of you raising your eyebrows, the lump appears to be arising from the nasal bones, perhaps in the region of the rhinion not the radix. In this case, smoothing the transition between the nasal bones and upper lateral cartilages may be most beneficial. My recommendation is to see a trusted rhinoplasty surgeon near you for an evaluation.

Although the prominence of your nasal spine may be bothering you, simply chiseling this down will not improve the relationship of your nose to your upper lip. You have what is called a "tension nose" caused by tenting of the skin and soft tissues from the top of your nose and septum. To get relaxation of these tissues, you would likely need a reduction of the dorsum combined with tip repositioning. This will create more of an acute columellar-lobule angle.

It is difficult to say without seeing images. One possibility is that there is usually some degree of asymmetry present before surgery that can become more obvious after surgery. My recommendation is to see your surgeon to determine what is causing the difference in appearance between the two sides and whether something can be done to correct this. They may want to wait a few more months until your nose is fully healed prior to any intervention.

Unfortunately, the role of facial muscle atrophy is quite small in relation to some of the other changes in volume and skin laxity. In fact, muscles actually work harder as we age in order to support and create expressions. This over activity is what creates the deeper lines around the mouth as well as in the glabella (between the eyebrows). If you are seeing some changes you want addressed, I would recommend considering a consultation to see what you would be a candidate for. Best of luck.

Sorry to hear that you are having that issue. Prolene sutures are not absorbable and it is possible to have inflammation around a suture that is under the skin. Sometimes if permanent sutures are inflamed or infected, they made need removal in order to correct the issue. You would want to seek a professional opinion with an in-person evaluation to determine the best course of action for you.

Thanks for your question. There isn't a right answer for this question. In my clinic, my first time patients are brought back in 2 weeks for an evaluation and any tweaks/enhancements are made at that time - which I cover. This is a case by case basis and determined on the initial consultation (ie, if I recommended a higher dose at the initial consultation but the patient wanted to start with less then there would be charge). It is hard to tell from the photos whether you have a drooping tip or just some infratip lobule fullness. Causes of infratip lobule fullness could include swelling or excess cartilage from the septum or lower lateral cartilages. An in person evaluation would definitely be helpful to determine whether this could be corrected with some simple maneuvers or whether you would need a full revision procedure.

Thank you for your question. Pictures would be helpful in this case. If there is still some eyelid retraction or ectropion then you could be experiencing some eye dryness and watering as a result. I would recommend an in person evaluation by an oculoplastic surgeon to determine the cause of your epiphora and whether surgery can help you. In the meantime, consider some moisturizing eye drops during the day and possible artificial tears ointment at night.

Happy to hear you are pleased with your blepharoplasty. I understand your concern, but lumps and bumps following blepharoplasty are very normal and settle out over time. I wouldn't rush to do anything at 3 weeks as this is still very early! If it doesn't settle out in 6-8 weeks, he may want to consider an intervention at that point.

Sorry to hear about everything you have been through. Unfortunately this is a tough problem without an easy solution. There is no data regarding prophylactic steroids and reducing scarring in rhinoplasty. I do treat scarring with injections of kenalog rather aggressively in patients that are forming scar tissue. As in every surgery, the risks of proceeding with surgery should be weighed with the benefits to determine whether another surgery is the right choice for you.

Hello and thank you for your question. It is difficult to say with certainty, but I think your skin thickness and shape would be amendable to a moderate amount of size reduction. The question would really be - how much? That is something that you would have to discuss with your surgeon to determine what is realistic.

Congratulations on your weight loss! You would definitely be a candidate for a direct necklift (with a midline scar) if you do not mind the scar. I recommend getting within 15 lbs of your goal weight first as you will likely continue to have more skin laxity as you lose more weight. In regards to your blood pressure, it is important that your blood pressure is well controlled prior to undergoing any elective procedure so that you can have an optimal outcome.
