Ivan, you can achieve a much better profile balance from appropriate reduction rhinoplasty along with alar base narrowing, and that alone may obviate the need for chin augmentation. Computer imaging with your rhinoplasty surgeon would  be most helpful in consultation.

Of course alar rim grafts can be surgically removed, but the question is whether you need them in the first place. Photos would be helpful, but as an alternative seek a second opinion from a board certified FPS or PS rhinoplasty expert.

I would advise strong consideration for rhinoplasty, as much more can be accomplished with permanent results. Fillers would mask the bump on your dorsum but only make the nose look too big anyway; and they are temporary! I advise consultation with a proven Rhinoplasty expert. You can discuss important aesthetic changes that you desire, such as reducing the dorsal hump, and others.

Neither procedure would be helpful, unfortunately. These are nodules of collagen, and may respond to needling to break them up, as well as injections of 5 FU and steroid mixed together. Multiple treatments may be needed, but they eventually shrink over time. Good luck!

I’d like to see a lateral photograph before a final recommendation, but it appears that you have a long upper lip and would benefit from this procedure. It would not affect your nose at all, so this would require additional rhinoplasty surgery. Recently techniques combining these procedures with fewer incisions have been published.

Your wishes for a more pleasing nose are indeed reasonable. Your nose is ptotic, overprojected, and with a prominent dorsal hump. The slight supratip break can be achieved as well. You should seek consultation with a rhinoplasty expert to start. Consider computer imaging as well as seeing results from similar cases as part of the consultation.

You can definitely achieve a straighter nose from rhinoplasty. Because there is severe deviation of both the cartilaginous and bony pyramids, and a severe septal deviation as well, a complete rhinoplasty with osteotomies, septoplasty, strong spreader grafts and midline anterior septal support would be necessary. Looking at your pre op photos and post op simulations, I think you have very reasonable expectations, and an achievable result that would closely match the improvements in dorsal reduction and tip rotation with slight deprojection that you desire. Endonasal approach would be fine for this surgery.

You do not need chin reduction or jawline sculpting. Based on the photos submitted, only a conservative rhinoplasty for dorsal hump reduction is all that I would recommend. Your tip projection and nasolabial angle are fine as is. Good luck!

First of all, you have what is referred to as alar columellar disproportion, with some hanging of the columellar and relative higher alar margins. There is a need for tip reshaping as well. A subtle reshaping of the nostrils can be achieved along with the above improvements is possible with endonasl or open approaches, and it depends on the skills of your surgeon. I advise consultation with an experienced FPS who is facile with both techniques.

All of the options you have been presented with are reasonable, but understand the surgeon will recommend the procedure that works best in his hands, most often. It makes sense to use autogenous fat for longevity, and my preference is for fat repositioning along with a transconjunctival approach. I routinely perform a fractional CO2 laser peel of the lower lid skin as well, rather the skin excision, in most cases.

You can accomplish the desired improvement with both procedures, but one fractional CO2 laser vs multiple RF microneedling treatments are necessary. Each has advantages and disadvantages, so consult a board certified surgeon who is knowledgeable.
