Thanks for your question. The issue of antibiotic use around the time of surgery is getting a lot more attention lately. For any kind of elective surgery, the general rules are, if you're going to use antibiotics, give a dose prior to the surgical incision and then use them for no more than 24 hours afterwards. Thanks for your question. You have a couple of issues. The first is that a muscle that attaches to your septum, the depressor septi, is very strong. This can be detached from your septum during surgery. That is responsible for the dynamic pull, but you also have an under-rotated nose to begin with. Thanks for your question. You're right. Rib cartilage can warp. However, there are studies that show that quite a lot of the warping happens in the first 20-30 minutes after harvest and carving. My standard way of handling the cartilage is to harvest it, carve the grafts, and then let the grafts soak in antibiotic solution for 20-30 minutes while I close the chest and readdress the nose. Thanks for your question. As long as you didn't do any permanent damage with the cocaine use, it shouldn't keep you from having surgery to help with your breathing. Thanks for your question. I see deviated septums all the time, but we only choose to operate if the patient is having difficulty breathing, worse on the deviated side. If you're using a prescription nasal spray and you're still having trouble breathing, it's time to talk to an ENT again. For thick skinned patients, the taping definitely helps to allow the skin to stick down to the new framework and helps limit swelling in the dorsum. You've mentioned plenty of good options for static lines and it basically depends on what you're willing to sign up for. Frequently, the result that they give in a revision rhinoplasty is much better than what your nose looked like before the revision.
