The term “alarplasty” means plastic surgery of the ala nasi or nostrils. However, practically speaking, it usually has meant narrowing or bringing in the nostrils or nasal base, if they appear too wide or flared. This may mean excising part of the nostril wall, the nasal floor or both. However, in recent times, because these narrowing procedures have been over-used, inappropriately performed or taken to unnatural extremes, alarplasty has more recently become a plastic surgical procedure to reverse the results of prior narrowing.

There are 5 main approaches that I have to accomplish this.

The simplest is expanding the nasal base using internal nasal flaps in a V to Y approach. This procedure usually lateralizes each side approximately 2-3mm and the scars are hidden within the nose. If more lateralization (widening) is desired, I use either a composite (skin + cartilage) ear graft to the nasal floor or advancement-rotation flaps from the upper lip. These procedures usually give 3-6mm per side of widening. A graft from each ear is needed per side, since the ideal ear graft cannot be divided in half to accomplish these results for both nostrils. The grafts within the nose produce hardly visible scars and the donor sites in the ear also heal quite well with imperceptible scars. Because there are risks of these grafts not “taking” (esp. if there is a moderate amount of scarring from the prior surgery), I have developed an alternative approach using an advancement-rotation flap from the upper lip. There is essentially no chance of this flap failing, but it may elevate the lateral upper lip slightly. This is usually another positive result in the right patient, but may not be desired in some patients.

If elevating the entire upper lip is also desired, a variation of the upper liplift can be performed to also widen the nasal base. This produces 3-6mm of widening per side.

There are situations from prior alar/nasal base narrowing where some of the lateral alar wall has been removed. This is especially true when the objective was to significantly reduce flaring. However, this also tends to straighten and lessen the nostril curvature, which may produce an unattractive or unnatural result. In these cases, it may be necessary to take a composite ear graft that is not flat like the one mentioned previously, but three dimensional. It is taken from a part of the ear called the antihelix and has its own risks and complications that can be explained in detail , if a patient is interested in pursuing this.

Finally, if a significant amount of widening is desired, a three dimensional Z-plasty can be performed by moving the nostril attachment outward while moving an adjacent flap of cheek skin inward. The procedure is also called a “flip, flop, flap” and can lateralize a nostril 1cm or more.

Your nose presents a number of interesting anatomical issues, which gives you the impression of being unaesthetic. It appears that your bump is probably at the lower end of the nasal bones. This could have resulted from inadequate reduction in this area or too much removal of the cartilaginous dorsum below (distal) to it. Without seeing the entire bridge, it is hard to say what should be done. If the bump is further reduced, it may contribute to a greater, unaesthetic depression. My best advice would be to see a nasal specialist to get his in-person opinion.

It's difficult to project what you would consider as a "perfect" endpoint. You do have a tad amount of excess nostril show, but I don't think that narrowing the nostrils via an alarplasty is the answer. I would suggest lowering the nostril rims slightly, and maybe even adding rims grafts Creating a slightly more aesthetic tip with a small graft is also a consideration.

Hello Alexandria. You have flaring of the nostrils which makes your nostrils appear too "open"! My approach would be to reduce the alar walls and not the nasal floors. This would reduce the flaring and decrease the amount of nostril "show" but not decrease the external valves or actual openings, which could cause some degree of nasal obstruction.

However, in general, there are many approaches that a more experienced surgeon may have to help you obtain your objective. You should seek opinions from surgeons who specialize in revision rhinoplasty.

Usually "shortening" of the nose means decreasing the length from the nasion (top of nose between eyes) to the nasal tip. The most common way of doing this is by rotating the tip upward. I am not sure what you mean by "heightening the full density" of the nose. You would be better served by adding some photos to your question and relating it to an endpoint you have in your mind. Other than that, it is hard to know what you mean.

It seems to me that either the left tip cartilage is bigger than the one on the right or the tip is being deflected to that side from a deviation at the end of the septum. In either case, it is something that will probably not improve with time and should have been corrected or prevented during your surgery!

Hello and sorry for your disappointing results. It is possible that the deformities resulted from dermal atrophy following the steroid injection. Occasionally this can be reversed by irrigating out the depot steroid with saline. Or they sometimes manifest as the post-operative swelling resolves. I am not sure why you removed the implant. But by so doing, the mentalis muscle dropped backwards and the vector of closing your mouth is probably at an angle now and not as vertical. Because it's at a less favorable angle, it has to work harder and hence the dimpling! If you make the tip more narrow & angular, you will be making the nostrils appear slightly more flared. So that is a decision that should be made following the tip-plasty. Or if there is doubt, I usually wait to see what the patient thinks and do it at a later time under local. It appears that the persistent left tip deviation is resulting in some, if not all, of the nostril asymmetries as well as the "mass" you see. You would best be served by getting a second opinion from surgeons with experience in revision rhinoplasty. It would have also helped to see a front view for a more thorough evaluation of the result.
