Facelift in Vancouver is a set of plastic operations aimed at eliminating age-related changes in the face and neck area by tightening the skin and subcutaneous musculo-fascial structures.
Rejuvenating operations also use lipofilling and / or liposuction of the face, movement and fixation of subcutaneous and deeper fat structures, which play an important role in shaping the facial contours.

The operation is performed through incisions located in natural folds (in front of the auricle), hidden from the eyes (behind the auricle, in the scalp) or along the hair growth line.
The essence of various facelift techniques is to move the soft tissues that have fallen in the process of aging, fix them and remove excess skin in the face and neck area.
Facelift and Neck Lift: Operation Planning
Indication for a facelift is the ptosis of the skin of the forehead and eyebrows, the lowering of the cheek area with a violation of the correct oval of the face, the deepening of the nasolabial folds and the exposure of the bone edge of the orbit, the violation of the contour of the lower jaw, and the formation of “jawls,” the sagging of the chin and neck.
These changes occur as a result of ptosis of the soft tissues of the face and neck and are accompanied by stretching of the skin, gradual formation and deepening of the skin folds.
When planning an operation, you can place your fingers in front of and below the auricle and, pushing them up and back, get a pretty good idea of what kind of result you can expect. As with other types of aesthetic operations, it is important that the patient has a realistic idea of the possible outcome.
It should be noted that the facelift does not affect the aesthetic characteristics of the skin. Therefore, deep nasolabial folds, folds around the lips to the inside of the nasolabial folds (“wrinkles”), around the chin require additional techniques that affect the skin itself (laser and chemical peeling or dermabrasion).
There are other factors that limit the possibility of achieving an ideal result. This is an anatomically smoothed angle between the neck and chin or an underdeveloped chin, which may require concomitant correction (increase) with the chin implant. The result is more obvious on the thinner faces with pronounced cheekbones, lower jaw and chin than on fuller faces and with a smoothing of the specified anatomical formations.
When planning a facelift surgery, it is important to have information about the patient’s diseases, especially those that lead to a worsening of blood circulation (microcirculation).
This is, first of all, atherosclerosis and diabetes mellitus. Also, harmful habits (smoking), for the same reasons can lead to poor blood circulation and, consequently, wound healing.
It is necessary to pay attention to the condition of the hair at the temples and behind the ears. The poor quality of hair in these areas can lead to a temporary loss of hair, which should be taken into account when planning the lines of surgical incisions.
Facelift: preparation for surgery
When preparing for surgery, the patient undergoes a standard examination. The examination is performed no more than 2 weeks before the operation. The examination includes a standard list of tests, ECG and consultation of the therapist.
Two weeks prior to surgery and a week after, it is necessary to stop taking acetylsalicylic acid and other drugs that may contribute to bleeding. Smoking patients should stop smoking two weeks before surgery for the reasons indicated above. The evening before the surgery, the patient should be thoroughly wash your face and hair, to-day operations do not have makeup on her face. 6 hours before surgery, you must exclude food intake and for 4 hours – fluids.

Facelift and neck lift: progress of the operation
The operation is performed under general anesthesia, which is supplemented by injecting a local anesthetic with a vasoconstrictor into the subcutaneous tissue to reduce sensitivity and bleeding tissues.
Depending on what parts of the face are more involved in age-related changes, the incisions are made according to pre-recorded markings in the temporal region, in front of the auricle, around the earlobe, behind the auricle and in the hairy part of the occiput above the neck. With the maximum volume of the operation, this line constitutes a single incision bordering the auricle.
With a limited volume of surgery, only the fragment from the indicated incision is used, corresponding to the area of the face that is supposed to be rejuvenated.
With moderate ptosis of the soft tissues of the middle zone of the face, which is manifested by their movement into the nasolabial fold and is accompanied by deepening of the fold and depletion of the soft tissue covering of the cheekbones and the lower edge of the orbit, provided that these changes are not accompanied by pronounced excess of the skin, micro-incisions are made in the temporal region, incisions on the eve of the mouth. It is possible to effectively raise the tissues of the middle zone of the face from these incisions with the use of endoscopic technique and fix it in a higher position inherent for the young face.
Such an operation, however, will not be effective with pronounced age changes accompanied by excess skin. It is widely practiced to carry out the incision located in front of the ear – behind the cohosh of the auricle, that is, practically in the outer part of the auditory canal. This modification of the incision allows to get a less noticeable scar in front of your ear.
With this method, the natural contour of the tragus of the auricle is somewhat disrupted, but this does not bother the patients, since this characteristic smearing of the tragus is very little noticeable.
The incision on the chin crease can additionally be used for pronounced age changes of the neck, which expands the surgeon’s ability to influence the subcutaneous muscle of the neck (platism). During facelift surgery, excess fatty tissue under the chin and on the neck is removed by liposuction. This greatly improves the contours of the neck. After the incisions, skin detachment is performed in the corresponding area of the face.
Earlier, the operation was limited to a tightening of the skin, but since 1970 the lifting the muscular aponeurotic system of the face, that is, a deep or SMAS facelift, has been increasingly applied.
A deep facelift in Vancouver is aimed at moving and fixing the system of superficial muscles of the face and neck in the direction opposite to the direction of gravity.
Directly under this layer pass the branches of the facial nerve, which provide a reduction of facial muscles. Therefore, the technique of these operations is much more complicated, and the duration is longer.

Facelift and neck lift: postoperative period
The early postoperative period is characterized by moderate soreness and a feeling of tension in the face and behind the ears. The first day after the operation, analgesics are used. Antibiotics are prescribed 5 days after the operation to prevent infectious complications. Swelling reach a peak at 2-3 days, and then gradually decline. Hematomas disappear after about 2-3 weeks. Moderate asymmetries of the face may persist due to uneven appearance of edema up to 2 months, and sometimes somewhat longer. It is also possible temporary weakness of the facial muscles, as a result of squeezing the small branches of the facial nerve with swelling or bruising, which takes place at the same time.
The dressing is removed on the 3rd day after the operation, after which the hair is washed with shampoo, dried with a hair dryer and the area of the surgical incisions is treated with an alcohol solution.
Rehabilitation procedures will help to achieve faster and more uniform dissolution of the edema (physiotherapy, hardware cosmetology), but, as a rule, this does not affect the final result of the operation.
The sutures are removed in several stages: partially from 7 to 10 days and finally (fixing sutures) on the 14-16 day.
The final result of the operation is estimated after about 6 months, however, the final appearance of the scars in the area of the incisions can be estimated 12-18 months after the operation.
In some cases, there may be hypertrophy of scars behind the auricle, which may require additional physiotherapeutic treatment and injections of hormonal drugs into the scar area. It is contraindicated to sunbathe 1.5-2 months after surgery as well as after most aesthetic operations.
Physical exercise is not recommended for the first 3 weeks after surgery. Patients may experience a temporary decrease in skin sensitivity on the cheeks and around the ears.
It lasts for 2-4 months, but some decrease in skin sensitivity may persist in the areas adjacent to the ear, but this discomfort usually passes quickly, especially against the background of those positive aesthetic changes that give such operations.
