I looked at your photo, and really, your nose doesn't appear that big. It just seems out of proportion to your other features. If you build up the jaw line and create a sharper angle at the corner of the jaw, all of your features will be more balanced. Sometimes I also build up the cheeks to, to also make a prominent nose appear smaller. Hard to tell from the one photo if you would benefit from that or not. I hope that is information is helpful.

Your lower eyelids appear to be very hollow with some bits of retained fat. Like other doctors have commented, it's hard to accurately assess your situation from the one photo in the upward gaze.

The easiest and most cosmetically desirable solution is carefully placed Restylane to the lower eyelids to restore the fullness and camouflage the fat.

Incidentally, you also seem to have a malar mount (pillow) on the right cheek which can also be camouflaged with filler.

Is is posisible to lower your upper eyelid scar, but it is not always so easy. It depends on the laxity (looseness) of your scar both above and below the incision, as well as the amount of fat (or lack thereof).

Regarding ways to reduce the amount of white showing beneath your eyes, there are various methods to raise the lower eyelids. The least option (other than doing nothing) is to try injecting filler to the lower eyelids. Sometime this is all that is needed.

Sometimes a canthoplasty will help. In this procedure, the lower eyelid is tightened at the outer corner of the eyelids. In your case and with your big eyes, I'm not sure that is the best treatment for you.

It is safe to use Latisse to grow longer, thicker, fuller eyelashes once you are fully healed. I recommend to my patients that they wait a month after upper eyelid surgery before beginning Latisse. It will take about 2 months for your eyelashes to grow -be sure to use it once a day for best results. (Using it more than once a day will not enhance your results).

It's hard to say without seeing you, but typically lower cheek fat can be removed or contoured using microliposuction. True "buccal fat" starts at about the level of the upper teeth near the mouth and goes up and outwards from there. Most people have fullness lower than that from the jowl fat pad or sometimes farther to the side from the malar fat pad.

Marionette lines refer to lines that are visible from the corners of the mouth, down towards the jaw line on either side of the chin. This is caused by a fold of cheek skin folding over around the mouth and usually this results from loss of fullness in the cheeks, or decreased skin elasticity, or both.

From your photos, it looks like what you want to achieve is an "epicanthal fold". This is a little web of tissue that overlaps the upper eyelid crease  and middle corner of the eye, near the nose. The simplest way to achieve this is by injecting a drop of filler in the upper eyelid. I do not recommend a surgical approach to correct this in your situation.

At your age, sagging around the mouth can be caused by a number of things. The most common ones are: loss of volume in the face (especially the cheeks) and loss of skin elasticity. Bone loss also can contribute to aging changes especially around the jaw and mouth.

If you go to an experienced practitioner, injecting Hyaluronidase in the nose to soften or dissolve the Restylane is safe. If you had the filler injected recently, you may still be able to mold it or flatten it out by gently pressing it with your fingers or a q-tip.
