Answer Snippet
I have a small chin. Could I increase the vertical height of my chin using fat transfer?
Autologous fat transfer to increase vertical length of the chin
Jan 11, 2023
Dear Cherry Jagar,
Thank you for your question. Vertical length can be created using fat transfer, but the dimension of length becomes more challenging the greater the vertical increase due to graft survival. Silicone (not liquid, but solid implants) can create a more uniform shape that has definition, feels like your skeleton and is non-mobile. Hope this help you.

Can a lip reduction to the center of the lip improve upper teeth show?
Lip reduction to improve smile
Jan 11, 2023
Dear an132,
Thank you for your question. Two other items that are helpful to know and see is how your lips rest (slightly open) and to feel if there is a large outcropping of bone right at the center of your nose. This would be at the base of the columella (this this the tissues right between the two nostrils). The reason for this is to see if your upper lip is restricted in motion by bone versus lip excess. I hope this helps you and wish you the best in the new year.

Can scalp Botox stop/alleviate hair growth?
Botox for hair loss
Apr 17, 2018
Dear Carlos.Dac,Thank you for your question.  There are a number of next generation therapies that are utilized for hair restoration/preservation.  These include PRP (platelet rich plasma), laser, and Botox.  Preliminary studies in the treatment of scalp sweating with Botox had serendipitous findings of hair growth/hair preservation.  The concept is that the Botox promotes vascular circulation to the relaxation of musculature.  This, of course, is a new field/application that is just being…
Considering Botox, how do I know if they are trained properly to inject?
However Y no if someone injects Botox well?
Apr 4, 2018
Dear SeattleChick, Thank you for your question. The question you ask is a very important one, and depending on who you ask – the answer may vary.  There are number of specialists who perform good work: Facial plastic surgeons, whole body plastic surgeons, dermatologists, and oculoplastic surgeons.  Ultimately, with appropriate training each of these individuals has the “education” to perform good work.  One of the more important elements though is the results of the said providers and how t…
I don’t like my chin. When I smile my chin sticks out and I would like to permanently fix it
Prominent chin with smiling
Jan 30, 2018
Dear heartbrie1998,Thank you for your question in the images you have provided.  The images that you have provided classically show “ptosis” of a chin fat pad.  What this means is that when you smile the fat pad rotates down and back, off of the “bony” chin of the jaw. The correction of this is a surgical procedure with meticulous reduction of the fat pad as well as reduction of skin along the chin. The scar is hidden under the chin and out of sight. If this is something that you wish to ad…

Unwanted facial changes after Botox placement – what to do?
Jan 30, 2018
Dear Gilligans,Thank you for your question and for sharing your images.  Although the first impression is to immediately try to create additional changes, I personally think this is a mistake. The contour changes that you see are related to too high of an injection of the Botox along the masseter muscle.  For your desired effect the Botox should be injected closer to the lower portion of the jaw – to create a more oval appearance of your face. In around 3-4 months after the current B…
Can a person still do autologous rhino who inject semi perm filler (aquamid) 5 years ago?
Nose surgery with permanent filler in place
Dec 17, 2017
Dear Cel2911, Aquamid is non-absorbable soft volume filler. Concerns in nasal surgery with such a product in place include, but are not limited to, infection of filler with need for additional surgery to remove all implant material, abnormal scarring and even tissue loss. The filler is not living, is unable to assist in the survival of autologous rhinoplasty, and most likely with create unforeseen complications, early or late. I am sorry to mention this to you, but these are the main co…

Pain-free Alar Reduction
Dec 17, 2017
Dear Jade7771,This is a very reasonable question.  In the circumstances I place topical anesthetic along the gums within the mouth and perform nerve blocks through these numb mucosal regions.  This will allow for complete numbing of the lower nose and a relatively painless procedure.  As noted additional caution, and for better hemostasis (bleeding control) I also injected local anesthetic in the region in which surgery will be performed. Of course, this is my own approach, and it would…
What type of nostril reduction is best for Asians?
Best techniques in Asian Rhinoplasty
Dec 17, 2017
Dear enquiryinquiry,Thank you for your question as it gives an opportunity to review that all noses do not begin at the same start point. Each nose is different as it relates to each individuals ancestry.  Often, patient’s wish to have the “ideal” version of their own nose.  Something that they observe as “self” versus nonself or “done”. When looking at the images you have provided there does not need to be any direct reduction of the nasal sidewalls as opposed to reduction and rotation of…
Ear Cartilage (DCF) or Silicone Implant for Asian Rhinoplasty?
Asian Rhinoplasty and Implant Choice
Dec 15, 2017
Dear Sanawm,Thank you for your question. From the photo you provided you have a nasal hump that would need to be reduced to give a nice and natural aesthetic line. Placement of a silicone 1.5 mm implant will only make the whole nasal bride higher. Infections are not related to thickness, they are related to biologic contamination and resulting biofilm formation (1.5 mm implants can become infected as well, but there is a lower chance of extrusion/breaking through the skin). If cartilage is p…
URGENT- Extrusion from only ear cartilage in tip from rhinoplasty 2 yrs ago?! Should I go to
Extrusion of nasal implant
Dec 15, 2017
Dear Gorgeousified,Thank you for your question. The question you ask is a very reasonable one and the short answer is that you should seek medical advice as soon as you can, preferably from the provider who performed your surgery. Extrusion of an implant can happen, but the AlloDerm that you had placed should be able to integrate into normal tissue and prevent such an event.  If there is other foreign material in the nasal tip, then extrusion can happen at any point in your life – often oc…
Planning to get a rhino, tip, alar surgery soon. How soon after surgery will I show possible
Nasal Implants
Dec 12, 2017
Dear BeeBoomBeeBoom,Thank you for your question.  The use of nasal implants has been long studied.  There are 2 different groups for nasal dorsum augmentation: Self and non-self.  In the “self” category this includes autologous rib cartilage, ear cartilage, septal cartilage, and fascia.  The use of these “own body” materials can be used in a variety of ways to naturally augment the nasal dorsum with limited adverse events.  In the “non-self” category we find non-organic implants (silico…
Should I look for an Asian nose specialist?
Specialist vs Mainstream
Dec 12, 2017
Dear Starheart,Thank you for your question.  The most important element in your search is to find a surgeon who specializes in nose surgery – not just an individual who is board certified.  Nearly 1/3 of my practice comprised of revising other surgeons work in the nose.

With regard to your Asian ancestry, your nose does not pose greater challenges been any other nose, but just specific challenges that inherent to your own.  The nose, as a whole, has the highest revision rate of any aes…

Asian Rhinoplasty. Am I able to change my nose to a high raised button nose?
Can I get a button nose?
Dec 12, 2017
Dear Kimiko94,Thank you for your question.  As I am certain you suspect one is not able to choose any nose and have it placed on your face, but there are certainly techniques and options that can refine your nose and bring its appearance closer to the images that you display.  You have a very good and strong bridge of your nose, which is good. Your nasal tip is more down-turned in the photos you display and your nasal skin is thicker than the images that have been provided.  Alteration in th…
Which rhinoplasty procedures should I get for the natural look?
Asian Rhinoplasty: What to do – or not
Dec 12, 2017
Dear Sparkle00,The question you ask is an excellent one – and you are doing it in an excellent forum (RealSelf).

Comment characteristics the patient see me for regarding a pure “Asian rhinoplasty” (no breathing issues) commonly involve treatment of the nasal bridge, the nasal tip, and the nasal width.  Not all patients need to each of these areas to be treated to have a very aesthetic and natural result, but I bring all these together see you have a more complete answer.
