Based on your photos, you appear to be a little young for a facelift. There is now a new minimally invasive procedure for skin tightening and fat reduction which uses radiofrequency energy to heat the tissues below the skin, resulting in fat reduction and improved laxity of the skin. The company that sells the device in the US is called Inmode and the particular device is called FaceTite. You may want to read more about it and inquire from your local plastic surgeons.

The superficial location of the swelling would render facelift or thread lifts useless in addressing this problem. I recommend treatment with injections of 5FU and dilute steroid solution.

I think that your surgeon is correct in assigning blame on your saliva glands in the area below your mandible. However, the left sided platysma band can be improved. Botox injections can soften the band. Minor surgery on the front aspect of the platysma muscle is another technique for alleviating the neck band.

I assume the second surgeon used cartilage from your septum or ear to repair the alar cartilages. It appears that you only had one rhinoplasty operation, that being 8 years ago and via a closed approach. Based on this description, I do not believe that you are at great risk for revision rhinoplasty. However, it is exceedingly important that you seek a surgeon who is experienced in revision rhinoplasty. The retraction may be due to placement of sutures in the columella or possibly due to the columellar incision (it seems that you had an open approach rhinoplasty). You are only two months after what seems to be a fairly major rhinoplasty. Clearly, your nose will require months of recovery before the final outcome of surgery is manifest. I do not recommend any further treatments until the nose has recovered further. Unfortunately, there are no simple techniques that would help you at this time.

Although you are unhappy with the current results, it is possible that with passage of time and resolution of swelling, you will start to like your nose more. Steroid injections in the tip can help expedite the resolution of swelling. The injections can still be effective after 9 months or even longer. If you continue to dislike the tip despite passage of more time, you may need revision surgery. I advise that you wait for another 6 months before pondering such treatment.

I think it is worth trying facial fillers and precise Botox or neurotoxin placement as an alternative to eyelid surgery. Hi, I think it’s difficult to comment on your particular concerns with the photos that you have supplied. Ideally, you should have someone else take the photographs and refrain from frowning so that your eyelids could be better assessed in a neutral pose, while looking directly into the camera. Having said that, I think that you are too young to be considering canthoplasty to canthopexy.
