I believe you have a seroma when you can feel a wave under your incision. You also have a drain site infection. You need to see your plastic surgeon to have your seroma drained and the infection treated.

The best way to figure out whether you have enough fat for both procedures is to estimate the amount of fat needed for each area and the amount of fat available from existing donor sites. I prefer not to use existing donor site more than once, because scar tissue forms after you have the area liposuctioned. It is always more difficult to get fat out of a scarred area. If you have very limited donor sites, then you should decide which area is of greater priority to you.

I do not think you will have any significant nicotine levels in your blood from smelling the cigarette smell in the walls. It sounds like you are very healthy and practice healthy lifestyle choices. I am sure you will do well with your tummy tuck. You may have developed allergies to something else. You may need to do some investigations on your own as to identification of the allergen.

Your asymmetry will change with time. Your right lifted and augmented side may relax and become saggier with time. Your left augmented but not lifted side may not relax to the same degree as your right side. The difference between your breasts has more to do with having more skin in the lower pole of your right breast, or more sag in your right breast. I would do a revision on the lift of your right breast to achieve symmetry and not change the implants.

I typically will use drains for my explantation patients to minimize any potential risk for complication, such as a seroma. Seromas prevent proper healing in the breast and chest wall tissues and could create more scarring and unpredictable results. I hope this helps.
