Answer Snippet
To fat graft or not with facelift?
Fat grafting with facelift
Jan 19, 2019
Excellent question - and there are many correct answers. Many facial plastic surgeons perform simultaneous fat grafting in the cheeks and under eye area to improve volume deficient regions that are not commonly improved with the facelift alone. You’re correct in that 30-70% of the fat will be resorbed as you heal. This may mean that an additional volumizing agent may be required in the future - but when done properly in experienced hands it provides a great result.
When getting rhinoplasty, I only had packing and a splint. Does not everyone get stents put in? What
Rhinoplasty aftercare
Jan 19, 2019
A splint and tape placed atop the nose is commonly done to tamp down swelling on the bridge and tip after surgery. Splints and/or packing inside the nose is done to both keep the septum midline during the healing process and prevent swelling inside the nose. However, which patient requires splints or packing depends on the surgery and surgeon preference. Not every patient needs stabilization on the outside and/or inside of the nose after surgery. Your...
Ethnic rhinoplasty open or closed?
Endonasal Rhinoplasty
Jan 16, 2019
A rhinoplasty performed without an external incision is called the “endonasal” technique. For many years this was the primary approach to rhinoplasty, although over the last few decades the pendulum has swung towards “open” rhinoplasty- which requires an incision in the skin between the nostrils. If the incision isn’t concerning to you, the change you’re hoping for can definitely be performed via the endonasal technique. When you visit with a rhinoplasty surgeon be sure to mention you...
Why are noses overly rotated up during/ after rhinoplasty?
Rotation after rhinoplasty
Jan 16, 2019
Great question - controlling the precise rotation of the tip after rhinoplasty is essential to getting a great result. No matter the technique, once the surgery is complete gravity has an effect and the rotation descends to some degree. Depending on the approach this may be a barely discernible amount to a large amount. Because of this, the nasal tip is often rotated higher at the conclusion of the procedure so that it will settle in the first 6 months to the ideal position.
When Did Swelling Go Down After Your Rhinoplasty Surgery?
Rhinoplasty recovery
Jan 16, 2019
The long recovery after an open rhinoplasty can be frustrating. The cast placed during the first week keeps the swelling down and the cartilage in place but once removed the nose expands. The swelling may go up and down as the weeks pass and you return to regular exercise and routine. It takes 6-12 months before you are able to clearly see the results.
What would you recommend I have done to fix my nose?
Rhinoplasty techniques
Jan 16, 2019
Great questions. A facial plastic surgeon should assess your nose and document your breathing difficulties. With this, improving your nasal breathing may be considered functional and possibly covered by your insurance plan. The most common cosmetic changes performed during rhinoplasty are 1. Reduction of a nasal “hump, 2. Straightening of the nose, 3. Narrowing of the tip of the nose and reducing the bulbous appearance. The latter two maneuvers may be helpful for you given your pictures. T...
Would rhinoplasty or possibly even a non surgical nose job give me a more feminine nose and profile?
Surgery vs. Injection Rhinoplasty
Jan 2, 2019
The benefits of injection rhinoplasty is that it requires almost no downtime, it’s more inexpensive, and -depending on the filler- is usually reversible. However it only augments rather than making true anatomical changes. The benefits are often subtle, it’s not without risk, and would unlikely to get you to your goals given the final photo you shared. Surgical rhinoplasty is very commonly performed but does require post-operative time to fully heal. It would be a more effective option to ...
Do I need a brow lift or a blepharoplasty?
Brow lift vs. blepharoplasty
Jan 2, 2019
Brow lift and blepharoplasty are both helpful procedures to improving asymmetry and providing both functional and aesthetic benefits around the eyes and brows. I agree with the prior comments that the first step for your concerns would likely be a non surgical procedure like Botox. Botox, Xeomin, or Dysport are all helpful for gently raising or lowering the eyebrows. You have a nice skin crease in the upper lids from the photos, and blepharoplasty can likely be avoided at this time. Thanks...
What are the best options to treat my turkey neck and double chin?
Neck rejuvenation
Jan 2, 2019
The previously provided answers may all be beneficial to different degrees, depending on your tolerance of downtime following the procedure, invasiveness, and price point. Three general categories are available - injectables, non surgical procedures, and surgery. Injectable options for you include Botox and is usually the easiest and least expensive option, but will have the mildest benefit. The procedural options include Ultherapy and Facetite - these are performed in clinic with...
Can a radix reduction and thinning be performed with closed rhinoplasty method?
Radix reduction
Oct 3, 2018
In short, the answer to your question is yes. Many rhinoplasty surgeons prefer endonasal vs open techniques depending on their comfort level with each approach, and most changes to the nose can be accomplished via either approach. The radix can be accessed through the nostrils without an incision, and can be narrowed, lowered or raised with this technique. All the changes you mentioned are possible. But each rhinoplasty surgeon has his or her own proprietary technique and can...
What procedure would be best to treat my scar and what are the chances it will be less visible? (Pho
Scar Revision
Sep 19, 2018
Thanks for the question. Based on your photo, you may be a good candidate for surgical revision of the scar. This involves carefully removing the scar and closing the wound in a way the scar heals without the depression, or concavity, in the skin. Sometimes dermabrasion or laser resurfacing after can be helpful as well.
I have a crooked nose and off-centered midline, what are my options to fix these issues?
Crooked nose
Sep 19, 2018
Thanks for your question. The nasal dorsum being slightly off-midline is one of the most common concerns for patients seeking nasal surgery. From. Your picture you may be a good candidate for “endonasal rhinoplasty”. With this, the nose is straightened through incisions hidden inside the nose.
Want to repair deviated septum, crooked nose. Since starting off with small nose, am I going to end
Deviated septum, crooked nose
May 16, 2018
Thanks for your question. Repairing your deviated septum, asymmetric dorsum and any other changes you’re hoping for does not require that your nose be increased in width. The changes you’ve mentioned are able to be achieved through both an open approach (with a small external incision) and a closed, endonasal approach (only incisions within the nostrils). I would visit a board certified Facial plastic surgeon to learn more regarding possible surgical repair.
Am I a good candidate for upper eyelid surgery? I am 31 years old and feel like my hooded eyes age m
Young blepharoplasty
May 16, 2018
Thanks for your question. I agree that often hooding of the eyelids can be caused both by natural aging - with descent of the brows and thinning of the skin - and hereditary. Blepharoplasty is often performed in younger patients for reasons just like you’ve mentioned in your question. A conservative, skin-only, blepharoplasty performed in clinic would be very helpful to achieve the appearance you’re hoping for.
Can rhinoplasty correct my specific flaws in my nose?
Customized rhinoplasty
Apr 27, 2018
Each rhinoplasty is customized for the specific concerns of each patient. Removing a bump from the nose is common, but is only one of many concerns many patients have with their nose. Your complaints of a round, wide and upturned nose can all be addressed by a qualified facial plastic surgeon.
