You seem to have minimal fat in the neck, no jowls, and a nice jawline; for that reason, my recommendation would be an anterior neck lift, using an incision placed underneath the chin. It is usually a bone or cartilage prominence that creates this obtuse angle in the junction between the lip and the nose. A simple procedure that will remove a portion of the bone (spina nasalis) or cartilage, or both, would take care of this and the columella base would retract. This can be done in local anesthesia. It appears that you have some residual filler which creates this lump. That happens occasionally after filler injection even if HA filler is used. If that is the case (this can be confirmed only with in-person consultation) removal of the filler is mandatory. The lower, transconjunctival blepharoplasty, to correct tear trough deformity, may be considered. Good luck.
