Statement: Babar Sultan, M.D., FACS, a double board certified facial plastic surgeon, was raised in Ellicott City, MD before attending Princeton University. There he graduated summa cum laude and as a member of Phi Beta Kappa. His interest in medicine and giving back to his community began from an early age, and he spent time as a volunteer EMT while in college. He subsequently spent a year working at the UCSF Carol Franc Buck Breast Care Center before attending Harvard Medical School. While surrounded by many pioneers of medicine, Dr. Sultan became inspired by the aesthetic and functional intricacies of the face and neck. Following medical school, Dr. Sultan began his Head and Neck Surgery residency training at the prestigious Johns Hopkins Department of Otolaryngology. He also continued pursuing his passion for scientific discovery by spending two years performing research under an NIH-funded T32 grant. During this time he authored several laboratory science and clinical papers in his areas of expertise.

After 7 years of intensive training at Johns Hopkins, he was accepted to the highly sought after fellowship in facial plastic and reconstructive surgery mentored by world-renowned facial cosmetic surgeon Stephen Perkins, M.D. During his fellowship, Dr. Sultan was able to perfect his skills in all aspects of the cosmetic and reconstructive surgery of the face, neck and scalp. Dr. Sultan is one of the few surgeons in the country who has gone through such rigorous and prestigious training, including board certification from the American Board of Otolaryngology – Head & Neck Surgery and a fellowship and board certification with the American Board of Facial Plastic & Reconstructive Surgery.

Having transitioned back to his home state, Dr. Sultan’s practice is exclusively dedicated to cosmetic and reconstructive procedures of the face and neck. Thank you for your question. You can certainly have a facelift after fillers. Its important to let your surgeon know the filler that you had and the location that it was placed. The same goes for radiofrequency treatments or medications such as accutane.

Thank you for your question. This likely represents a granuloma. Granulomas or inflammatory tissues require a source of stimulation, in your case, likely a deep suture. Your wound should be probed as well as cultured.

Thank you for your question. I think a kenalog injection is appropriate if the surgeon thinks the new small bump represents scar tissue. If the bump goes down from the injection then it should likely be a permanent result.

Thank you for your question. Without examining you in person, I can’t give my opinion without a qualifier. I certainly don’t think you need a facelift, but fillers in the midface might give you the shape or “lift” that you are desiring. I can’t tell the quality of your skin from the pictures but sometimes a laser treatment such as Halo can make the face look more refreshed.

Is my rhinoplasty officially botched? Will it collapse?

Having a rhinoplasty is a big deal in the sense of emotional and monetary investment let alone the physical healing. 1 month post-op is very early and I would allow some time for more healing. Seeing pre-op pictures as well as the operative note would help guide specific comments regarding potential future improvement. In the meantime, maintain close follow up with your surgeon and voice your concerns.

After reviewing your pictures, you are not a candidate for a facelift. If you feel that your lateral lips are thin then a little filler will be helpful. Overall, I think you look great. If you feel tired in your appearance, great skin care, exercise and healthy diet might improve your outlook.

An open reduction is where the nasal bones are broken in a controlled fashion to correct bony deviation or cause narrowing. An open reduction can be a component of a open rhinoplasty. A rhinoplasty or nose job can address other issues as well such as the tip or projection.

Thank you for your question. Without pictures or a time frame after surgery, it is difficult to answer your question. I recommend you call your surgeon to get follow up. I have my patients stop smoking 8 weeks prior to surgery because of the risk of skin necrosis.

Does massage really help get rid of lumps? What else can I do?

When one separates the different layers of the face/neck, the initial healing response has some scarring which can result in lumps and bumps. Time, massage, and occasional steroid injections will help those resolve. It is important to rule out a fluid collection.
