The picture on the left shows that quite a bit of the base of your penis is hidden by the skin and fat. You have a micropenis that is retracted. Possibly helpful to do the following: 1. Remove the fat and skin in the pubic area which are covering your penis, 2. Detach the suspensory ligaments which may be holding back the penis, 3.Put a sleeve to wrap around the penis shaft which may/may not lessen the retraction, 4. treat post-operatively with some sort of extension device.

It depends on whether your penis is hidden by the fat and skin. If so, removing the fat and skin will expose the penis more and looks longer but not actually longer. Sometimes the penis shaft is actually buried underneath the skin and fat, in which case, it can be "unburied" and the penis would actually be longer.

From your description it sounds like you do have a micropenis. Treatment would depend on many different factors. You may have a good sized penis but most of it could be buried inside the skin and fat.

Yes, that is scrotal webbing. Normally the junction between the scrotum and the penis shaft should be at the base of the penis shaft. In your case it is attached almost at the tip of the shaft. This make the penis much shorter visually and for functional purposes during sexual activities. It could be easily excised but be sure your surgeon do a Z-plasty to prevent bowstring contracture. Your surgeon will know about this.

The fat in the pubic area and the scrotal webbing are definitely making the effective length of your penis shorter both visually and functionally. The pubic fat and drooping skin could be corrected relatively easily with liposuction and skin lift. At the same time the scrotal webbing can be excised and a plastic surgical procedure called Z-plasty should be done at the same time to prevent a "bowstring contracture"

What you describe is relatively common. Removing the fat by liposuction and lifting the pubic skin surgically can be done relatively easily but there will be some scarring which can be minimize by careful surgical techniques. This is something plastic surgeons do frequently.
