Based on the folds you have, liposuction alone would not be sufficient and would leave you with excess skin.  Combining liposuction to the entire midsection, front and back, with a tummy tuck, would be the only way to get the flat stomach you desire.

Although always a risk when performing a breast augmentation, it can be minimized with appropriate planning and technique.  Lowering the fold definitely increases that risk; therefore, it should only be done in the appropriate circumstances as other techniques must be used in that scenario to minimize that from happening after surgery.  Larger implants definitely increase that risk.

If you are terrified of capsular contracture and sitting to high then my recommendation would be under the muscle.  You also appear to be deflated enough (can only tell on examination ultimately) that rippling may be evident if you went above the muscle.

Its possible to address this area and any others through a closed approach, however definitely need to discuss with a board certified surgeon the pros and cons to both an open and a closed approach but more importantly the outcome....many times just altering one aspect will change another if its not properly addressed at the time of surgery.  The final outcome you are looking for is much more important than open vs closed approach.
