The “indents” are a normal anatomic area of the nose referred to as the soft tissue triangle. The size and shape of this area is determined by the size, shape and orientation of the lower lateral cartilages. The lower lateral cartilages can be controlled with a combination of sutures, grafts and partial excision in rhinoplasty. Filler would not improve this much. You are better off having revision surgery to correct the domal angle, caudal excess and hanging columella.

Yes, a natural result is possible with a closed approach. You have what is known as a tension nose. This is when there is a convexity from the bridge all the way down to the upper lip. Possibly. The columella is composed of skin, cartilage and some connective tissue important for structural support of the nasal tip. The surgeon will need to be sure that manipulating these structures will not negatively impact the tip shape or support.
