Can I be a candidate for just tip plasty?
Tip rhinoplasty
Dec 21, 2020
Thank you for the interesting question. There are many things that can be done to improve the appearance of your nose while concentrating on your tip. The cartilage of the tip can be reduced and reshaped to improve the dome and tip defining point. Additionally, a graft on the tip will raise it and reduce the appearance of the dorsal hump. The upper third of your nose appears thin and therefore there would be no reason to break and reshape your bones. Consultation with a board certified...
What can I do about a bone that sticks out on the left nostril?
Caudal Deflection
Dec 20, 2020
Thank you for you interesting question. The septum is a plate of cartilage that separates the two halves of you nasal passages. The most inferior portion (caudal) of you septum is clearly deviated into the right side. This can affect breathing and the external appearance of your nose. A complete examination can help determine the condition of the remaining septum as deflection further up commonly affect breathing. Consultation with a board certified plastic surgeon would allow you to...
Filler or surgery? What procedures do you recommend?
Fillers or Surgery?
Dec 19, 2020
Thank you for the excellent question. Your best results would probably be achieved through a combination of fillers and surgery. Correction of jowls and loose skin in the neck would best be corrected by lifting and tightening your skin. This would improve your jaw line and profile. Placing fillers in this area would at best give the appearance of gaining weight. Lifting would also correct positioning of tissue in the cheek improving the appearance of loss of tissue in your lower eyelids that...
My ears are pointy and stick out - Can these be rectified with surgery? If so, how?
Prominent Ear
Jun 7, 2020
Thanks for the great question. You appear to have two separate complaints; pointed tip and prominent ear. If you think of the ear anatomy as two separate components, then ways to correct your concerns may become clearer. The central part of the ear (Concha) is bowl shaped and is surrounded by a "flat" portion (Scapha). Inside the edge (helix) of the scapha in a fold called the antihelix. This fold usually divides into two folds (superior and inferior crus) superiorly forming a...
I'm planning on having a neck/facelift this summer or fall. What other procedures do you recom
Options with facelift
May 17, 2020
Thank you for the excellent question. A full facelift with repositioning of the cheek tissues, liposuction of the neck and jaw line and repositioning (tightening) of the neck muscles would give you an excellent result. You do appear to have asymmetrical positioning of your brows (right higher than left) resulting in asymmetrical appearance of your upper eyelids which could be corrected with endotine brow lift, if that bothers you. Good luck, hope all goes well.
What do you recommend to lift my jowls between a facelift, neck lift, or other?
Face versus neck lift
May 17, 2020
Thank you for the excellent question. Your best result would be a facelift. A properly performed facelift repositions tissue from a plane through your pupils down into and including your neck. As cheek tissue descends with gravity and aging the effect is of a loss of tissue in the lower eyelids with hollowing which you can just see in the first picture. The cheek tissue changes from a gentle "C" lying on it's back to a "V" as can be seen in the first and third picture. This pushes...
What would you recommend to improve my face?
Facial Options
May 17, 2020
Thank you for the excellent question. From you photos, you appear to have fat that could be removed via liposuction to improve your neck and jawline. This could be improved with a facelift to reposition your cheek tissue and correct early jowl formation while repositioning skin to help shape the lower two thirds of you face. A personal consultation with a board certified surgeon can help you with your options. Good luck!
If you’ve lost trust with your surgeon, is it possible to consult with other doctors?
Second Opinions
May 17, 2020
Thank you for the excellent question. There are many reasons to return to your original surgeon including a working knowledge of your original goals and starting point resulting in your original consultation, knowledge of the original procedure performed to achieve your goals and presumed commitment to achieving your goals resulting in consideration towards reducing further financial burden if any touchup procedures or manipulations are required. Having said that, second opinions...
Would a facelift/thread lifts be enough to give me some symmetry?
Facial Asymmetry
May 17, 2020
Thank you four your excellent question. Everybody has some asymmetry which can have multiple causes. From your photographs I believe that non-surgical treatment including soft tissue fillers including fat combined with minimal liposuction and skin tightening would give you the result that you desire while avoiding a facelift.
How long should I wait after Botox, threads and fillers for a facelift?
Facelift after non-surgical facial rejuvenation
May 17, 2020
Botox generally would not alter the design and planning of a face lift. With fillers, it would be best to delay until they have attenuated to facilitate planning and consider fat grafting. If considering a facelift it is best to avoid threads, as I have found that they can definitely affect the procedure and ultimate result. Thanks for the question.
Can weight loss of only 10 pounds reduce volume in face?
Facial aging with volume loss
May 10, 2020
People tend to deposit and lose fat in preferred areas which varies widely by sex and individual. Hormones definitely plays a part in this process. There are interesting studies of twins which demonstrate that a fuller face is perceived as younger. Replacement via fat grafting might be an excellent option for you although, at 51, you might also benefit from lifting as well. A consultation with a board certified surgeon will help direct you to your best options.
How long does a midface lift in which the cheek fat pads are lifted by a suture last?
Midface lifting
May 10, 2020
Thank you for the excellent question. Repositioning of the tissues of the cheek are part of most lifting techniques. Results are generally improved with mobilization and repositioning of the overlying skin. The longevity of any procedure is dependent on multiple factors, both intrinsic (people age at different rates, dependent on inheritance) and extrinsic (environmental factors that we are exposed to such as smoking, sun, stress etc.). Additionally aging does not necessarily occur at a...
What are the most effective procedures available to me to improve/lift my bottom-heavy face an
Options for lower face
May 10, 2020
Thank you for your question. I do not believe that lifting would give you the result that you desire. Liposuction of the neck and jawline would improve the lower face and neck while filler in the cheeks would enhance your facial proportions. Laser resurfacing would smooth and improve your external appearance and a skin maintenance program would help. Consultation with a board certified surgeon can provide a better understanding of the options available. Good luck!
What are some things that'll be very useful to me during my tummy tuck recovery?
Useful things to know after Tummy Tuck
Mar 7, 2020
Thanks for the question. I'm sure that your surgeon will go over your post operative care in detail. There are three simple things to do during this period which will greatly help in the recovery. I refer to these as the three W's as follows:
1. Wind: It is important to take frequent deep sigh breaths to prevent atelectasis and associated temperatures. Patients tend to suppress these sigh breaths after surgery due to the presence of pain. Remaining upright as...
My sagging skin is above my belly button after losing 60 lbs. Will a tummy tuck still work? (p
Correct tummy tuck with excess above belly button
Mar 6, 2020
Thanks for the excellent question. To correct excess above the belly button you should consider a full tummy tuck. In this procedure an incision is placed around your belly button allowing it to stay in its normal location. The tissue above the belly button is then mobilized allowing it to be repositioned inferiorly. Generally all of the skin and fat between the lower incision and the belly button is removed and a new opening is made for your belly button. Your other concern pertains to...
