Your question assumes a "one size fits all approach to surgery". Patients with fat faces due to thick skin and deeper tissues will benefit from techniques that remove SMAS. Patients who are very thin need to save all the subcutaneous tissue possible so they won't look like walking skeletons. Techniques that excise or spare the SMAS can both achieve the same degree of repositioning.

Reducing the dorsal structures will almost always make a tip loose projection especially one that is already drooping. Dorsal and tip surgery can be done via either an open or closed approach.

Botox or Dysport will cause the muscle to relax and may help the grinding. Maintaining the treatments will also reduce the thickness of the muscle. If the muscle is contributing to the facial fullness then the treatments would also help the appearance.  It appears that reduction of facial fat would also reduce the size. An exam is needed to give a definitive recommendation of liposuction or fat pad reduction.

Of course you can but the real question is should you. Unfortunately the answer is that even in the best of situations the trade-off of the scar in a conspicuous location for a minor asymmetry is just not worth it.
