Doctor’s answers

Question
Answer Header & Date
Answer Snippet
Can I use makeup 3 days after CO2?
No makeup until 10 days after the procedure
Jun 28, 2018
I always tell my patients to wait until 10 days after the procedure before placing make-up on the healing skin.  Darkness to the skin directly after the procedure generally is desquamation of the epidermis.  The first several days are the most challenging, but swelling and discoloration indicates that you had a good treatment
Indentation in chin 6 months after traditional facelift
The chin can be fixed
Jun 28, 2018
The marginal mandibular branch of the facial nerve is at risk in facelift surgery.  Permanent Injury is rare, but resolution of the problem can occur up to 1 year after the surgery.  After looking at your before pictures it is not surprising that the under the neck area still has fullness issues.  You started off with a lot in this area.  This can be significantly improved, even fixed with a revision procedure which focuses on reduction of bulky residual tissue and then tightening of the dee…
Why does my right eye droop?
Face asymmetry
Jun 28, 2018
Essentially we all have facial asymmetry and certain differences are more prominent than others.  Both your eyebrows are sagging, and the one on the right is sagging more.  Like everything else, the forehead also can sag causing overhand over the upper orbital rim.  It is natural to have asymmetry, it comes with the sagging that comes with aging.  The answer is to have a browlift so as to elevate the brows higher, and elevate the right higher than the left.  Upper blepharoplasty will also …
Can you tell anything about my septum and nasal bones from these slides of a ct scan? (Phtoo)
CT scan discussion
Jun 28, 2018
The asymmetry seen on your picture is due to a small degree of greater medial displacement of primarily lower lateral cartilages, and a small degree of greater displacement of the nasal bones inwardly.  The CT scan is not generally needed for cosmetic rhinoplasty, and I use if only for sinus concerns. What I can see indicates nasal bone inward displacement on both sides, the septum is reasonably straight, the right inferior turbinate is small anteriorly.  I would consider filler corre…
Do injections help maintain a facelift?
Staying young
Jun 27, 2018
There is no question that a facelift can provide one of the most significant rejuvenations for your face.  But what is also important is the ongoing maintenance that keeps things going in the right youthful direction.  Comprehensive simple effort makes a big difference.  Skin care, facials, microneedling, botox, fillers, all help to maintain youth. Of course, it goes without saying to keep yourself away from smoking, sunshine, and stress; these are youth killers.
Filler or Facelift?
Filler
Jun 27, 2018
The big picture here in my view is that you have great features and anything done is to try to subtly make enhancements and artistic acentuations.  Facelift is not a good answer for you. Filler can accentuate some of your features such as sub malar and malar regions.  Laser lipolysis can provide strategic reduction of fullness and can add some tightening.  Be careful with anything that you decide upon.
Would Fractional Laser treatment help my deep acne scars?
Some improvement
Jun 18, 2018
Deep acne scar changes are difficult to get resolve completely, however, a deep fractionated CO2 laser treatment can provide some decent improvement.  The expectation is for improvement, but do not expect that the marks will gone.  Often the more superficial scars can be improved the most.
What do I actually need done? I want to just fix my eyelids, but was suggested a face and neck lift
Many options
Jun 18, 2018
Of course you don’t need to have anything done as this is a cosmetic treatment.  But, in order to look at this in a priority fashion, I favor upper blepharoplsty as the best option to achieve the best bang for your effort.  Next, a facelift with liposuction would provide a good result. Third most important would be a lower blepharoplasty.  In my experience, the more treatments that you do the greater the satisfaction when all has healed.
Tight and stiff 8 months after facelift
More time
May 31, 2018
The tight sensation can take a long time to resolve in certain instances.  Most of the time tightness lasts a few weeks or months.  However, there are instances whereby the tightness lasts for more than a year.  Massage, ultrasound, steroid injections, are options for treatment depending on the particular circumstance.
Thread lift vs actual facelift. Advice?
Larger effort provides better long-term results
May 31, 2018
Thread lifts can provide some short-term benefits while providing the non-surgical option.  Facelift is the best answer to achieve the rejuvenation that most individuals are hoping for and expecting.
2 weeks out from botched facelift, what should I do?
Skin necrosis
May 31, 2018
The tissue in front of the ear was elevated during the surgery and it now has a very compromised blood supply rendering necrosis of the tissue.  The most common cause is related to smoking but other associations include medical conditions such as diabetes and cardiac disease. The best answer now is to get good support from an expert doctor who is expert at taking care of this problem.  Your facelift doctor is your first choice, however, if he has hesitation related to taking care of this p…
Do all facelifts include Platysmaplasty?
Most facelift surgery requires a platysmaplasty for optimum results
May 6, 2018
Not all individual patients who undergo facelift surgery are provided a platysmaplasty, however, the majority of patients would benefit from one in order to achieve optimum results.  The platysma bands noted in the pictures will only be rectified if a platysmaplasty is performed.  In most instances this requires a small incision under the chin for access to the platysma.
Neck lift lower facelift: Is tightness normal 4 days post-op?
Hardness is normal
May 6, 2018
Yes this is normal.  Underneath the skin the deeper plane of fascia and muscle were tightened and at a 2 to 7 day period post-operatively the tissue can become somewhat hard.  This is due to the healing undertneath, and due to the overall healing process.  After a few days the hardness will subside.  Ultimately the tissues we soften and appear and feel like normal soft tissue.
Is it time for a facelift?
Yes, a mini-lift would benefit
Dec 5, 2017
Overall teh changes to the face neck and jowls indicate early aging, so you would be a good candidate for a mini-lift style facelift.  With this technique you can make changes far better than what ultherapy can achieve, and smaller incisions and less dissection can be performed.  The overall goal is to reduce the jowls, and lower face sagging.
Mid face lift OR Lower Blepharoplasty Or Cheek Implants?
Fat transfer is a consideration
Dec 1, 2017
I favor fat transfer to the face specifically to the areas of concern.  Start with a modest amount of filling to rejuvenate and contour, and if needed come back for additional filling.  The cheeks and lower eyelid region, tear trough, can all be sculpted according to your desires. I think it does  better than mid face lift or other options.
