Based on your picture, it does not appear that you have true keloids.  Your scar appears widened and possibly slightly thickened.  This could be due to the way it was closed rather than something else.  Without knowing how your doctor closed his incision, I would recommend re-excising the scar and closing it carefully in layers.  If it becomes thickened, injections of steroids could help.

Based on your pictures, it looks like your right implant migrates into your armpit when you are lying down.  This can be repaired using sutures inside the breast pocket to tighten up the lateral aspect of the pocket (capsulorrhaphy).  If necessary, additional support to the repair can be added using acellular dermal matrix (ADM) such as Surgimend, Alloderm, etc.  The downside of using ADM is that the stuff is really expensive.  I have found that my repairs without the addition of ADM ten…

Implants are pretty tough and durable.  They will withstand anything that a normal breast can withstand.  In fact, one of my colleagues has a video of himself driving his car over implants without them rupturing.  That is a pretty good stress test.  Still, implants are not stainless steel.  Eventually, they may wear out or, if you get hit just right, they may rupture.  Don’t live your life around the fear that your implants could rupture.  If they do, it is a relatively easy matter to replac…

From your “before” picture, it looks like you have a fairly wide cleavage.  Cleavage is determined by the width of your chest bone.  I have seen patients come into my office where someone has tried to give them better cleavage and bad things happened.  First, there is not much coverage over the chest bone other than skin.  If there are any ripples or wrinkles, they will be noticeable in the midline.  Second, and this appears to be what has happened in your case, the implants are not cent…

I would agree with others that you appear to have a “double bubble” appearance to your breasts.  The simple solution is simply to reconstruct the fold slightly higher using stitches in the pocket (capsulorrhaphy).  If your tissues are weak, you could reinforce this reconstruction using ADM such as Surgimend, Alloderm, Galaflex, etc.  I recommend that you see a board-certified plastic surgeon who can examine you in person and review your options with you.

A breast augmentation will add more volume to your breasts but it will do nothing to lift them.  That requires a breast lift procedure.  As you mentioned, the trade-off is scarring on the breasts.  Based on your pictures, I would say that you need a breast lift if you want a perkier shape to your breasts.  Implants alone will not do it for you.  I recommend that you visit a board-certified plastic surgeon who can examine you in person and review your options with you.

Based on your picture, it appears that you have an excellent result.  It sounds like you have the optimum situation with regards to your implants – silicone gel, under the muscle.  If you still have rippling and little tissue coverage then you might need to add tissue – acellular dermal matrix (ADM) or fat – to give you more coverage.  I would recommend that you discuss this with your plastic surgeon.

A donut lift is VERY limited in its usefulness.  It is only good for elevating the nipple/areola one to two centimeters.  I have done them under local anesthesia in the office previously.  Without a picture or an examination, it is impossible to tell if this procedure would be appropriate for you.  I recommend that you schedule a consultation with a board-certified plastic surgeon who can examine you in person and review your options with you.

Implants will add volume but they will not lift your breasts.  Your nipples are currently pointing towards the floor and they will continue to point downwards unless you do a lift in conjunction with your augmentation.  I recommend that you schedule an appointment with a board-certified plastic surgeon who can examine you in person and review your options with you.

Metal piercings are removed during surgery because an electric cautery is used.  Removing metal piercings prevents burns.  Since the nipple/areola remains intact and attached to your breast tissue, your nipple rings should be able to be replaced before you wake up.

Different doctors have different opinions on whether to combine a lift and implants in one procedure or separate them into two.  Personally, I like to do them in one operation.  It is less expensive for the patient and there is only one recovery period rather than two.  I have not seen that combining the two into one procedure has been detrimental in any way in my practice.  You might consider seeking a second or third opinion from another board-certified plastic surgeon.

Based on your pictures, it does appear that you have aspects of tuberous breasts in place, including the wide areolae and narrow base.  I think that you will need a breast lift procedure for better shape as well as making the areolae smaller.  This could be done with or without the addition of implants for volume.  As for the cleavage, you have a wide chest bone.  I don’t think implants alone will give you better cleavage and any attempt to do so will result in implants centered too near…
