Yes these procedures can definitely be combined in one operation. The advantages of this include having one recovery period to go through and probably a reduction in cost. The disadvantages include a longer surgery time however it has been shown to be safe to do these types of procedures together as long as you are healthy and medically cleared for surgery.

In general a lip lift aims to shorten the distance between the nostril and the red part of the lip. This will not make the philtrum fuller but it will definitely decrease the length. If the lips are sill need more fullness, fat grafting could be combined with this lip lift. If the corners of the mouth are already angled downward than this could be made worse with a lip lift and may need a corner mouth lift in addition to a lip lift.

A high SMAS specifically means that the upper border of the SMAS flap is started higher, closer to the upper edge of the cheek bone which allows for some lifting of the cheek and lower eyelid. A low SMAS flap will just affect the area around the mouth and jowl and not include the cheek. Deep plan and extended SMAS are more descriptive terms that are used to mean different things by different surgeons and are not synonymous with high SMAS flap.

Even if the first procedure was done poorly, often a revision can still be performed with significant improvement. Yes what you say is generally considered ideal for a caucasian.  However each individual is very different and there are many exceptions.  So for instance someone may still have a very pretty nose but have nostrils that are wider than the width between the inner parts of the eyes, and vice versa.  Ultimately these ideals need to be looked at with an aesthetic eye to determine if they are correct for each individual.
