Lip lines can be treated several ways. Laser resurfacing is the gold standard for removing a majority of vertical lines permanently. I sometimes combine this with dermabrasion or sanding as is described in my manuscript published in Plastic and Reconstructive Surgery entitled "Laserbrasion." I have utilized this technique for over a decade with great results. For non invasive treatments of lip lines Botox and filler can sometimes temporarily suffice.

You have early platysma muscle loosening , causing the start of neck bands, At age 39 a minilift should suffice to tighten these muscle and restore a cleaner jawline. Sometimes platysma corset sutures are also needed to firm up a lax neck.

Vertical banding of the neck is due to a lax platysma muscle. This muscle is tightened with a corset suture to reduce the bands and smooth the neck contour. the surgery can be combined with a facelift if needed.

A blepharoplasty can help to reduce bags. Using a hidden incision inside the eyelid the fat can be recontoured. You seem to have a tear trough groove, which is the deeper indent between your lid and nose. This area can be addressed by placing or sliding lid fat under the indent to smooth it. The result should be be smoother, more youthful lid.Filler can also be placed non surgically to the junction between the lid and cheek to soften this transition, but the results will not be as impressive...

If the lower lid bags are a concern, then yes have surgery. You asked if there were alternatives in another question. Filler can be placed around the lids to soften the circles, but I believe you are best served by surgery. Lower lid blepharoplasty surgery takes about 20 minutes and healing is 10 days. it should be painless, but you will have bruising. The results typically last 10 years.

Lower lid surgery can be redone. As we age the lids change and may need some revisions after 7 years. tissue becomes slack and a re-freshener is not unreasonable. Secondary surgery can be more challenging, so picking an experienced surgeon is important.
If a patient already has moderate to severe dry eye with plugs on all ducts, is surgery a risk?

Worsening Dry Eyes is a risk of blepharoplasty. If the puncta are plugged and the corneas look healthy on slit lamp exam with a good tear lab osmolality test, then conservative surgery may be possible. If you suffer from extreme irritation now then I would avoid lid surgery as this may worsen it.
