Caudal septal deviation and nasal turbinate reduction (cold coblation)
Deviated Caudal septum
Mar 12, 2016
It cannot be determined from these images what the precise etiology of your nasal obstruction is. There is even a possibility that you have allergic or vasomotor rhinitis as the cause given your complaint of discharge and turbinate congestion. Your photos only show some curvature of your medial crura footplates.A thorough exam should provide important information that will guide the surgeon to make the correct choices. If, in fact, your caudal septum requires realignment, that is usually...
My upturned nose and nostrils. Is it possible to make my nostrils smaller?
Prettier tip
Jan 18, 2016
Yes, it is possible to create those changes to your nasal tip. The position of the tip often affects the flare of the nostrils at the base and modification can be done to create a better balance between the two. At consultation you should be able to discuss the precise changes you would like to make and have a discussion with your surgeon about what would be involved with attaining those goals. It seems as though you are specific on what you would like to achieve and that makes you a good...

Dorsal hump resection and grafting
Jan 17, 2016
First off, I feel it would be best for you to simply focus on what you wish your nose to end up looking like. Leave it up to your surgeon to figure out how to get you to your goal and to determine if grafts are necessary. It is not common for any hump resection to create a need for grafting but I am wondering why you seem to be convinced that you don't want a graft and that grafts are bad or dissolve? Grafts do not dissolve nor are they something to shy away from if the benefits of...

Revision Rhinoplasty
Jan 17, 2016
Without knowing what your nose looked like before your last surgery it is difficult to say whether the benefits of another surgery will be worthwhile. Weighing the risks and benefits would also depend on precisely what was done at your last operation. I can see the outline of a graft in your tip and it might be best to remove that graft in order to debulk your tip (but the shape of your original domes are like might require some type of camouflage to remain in place.) The droopy tip must be...

Post rhinoplasty results
Jan 15, 2016
Even if you were not pregnant, seven months is not enough time to appreciate the final outcome of a rhinoplasty as things continue to change for many more months and in particular the nose becomes more narrow at the tip. Pregnancy creates a much more swollen soft tissue envelope and often a congested internal nose as well (rhinitis of pregnancy is a common condition). At this time I think it best to put your nose out of your mind and to revisit the issue when you are nearly done breast...
I do not want the scares at my ear/ temple area from a Face lift. Any suggestions ?
Facelift scars
Jan 15, 2016
There are options for where the incisions are placed when performing facelifts and this should be discussed with your surgeon at consultation as there are pros and cons to each choice. Most healthy, non smokers heal extremely well and certain techniques such as a deep plane rhytidectomy minimize the tension from the skin and serve to create even better outcomes with respect to the incisions.
Columellar stut problem
Columellar Strut
Jan 14, 2016
Based only on what you have said here it is likely that the existing strut was placed there to provide support. Sometimes the strut can be a bit too long and this can create the symptoms you are having. The solution depends on several factors including whether or not you need the strut at all, whether you need increased tip projection, how much columellar show is present and the symmetry of your domes. Your surgeon will be able to address your questions and resolve these with a relatively...

The ultimate goal of any cosmetic procedure is for the patient to be content and feel better about their appearance. The photos you've posted here are limited so that I cannot assess things well and render an opinion based on them, however you are dissatisfied and quite specific about what is bothering you and this usually makes for a good surgical candidate.. I advise you to return to your initial surgeon unless you've lost confidence in them to discuss your concerns. If that visit leaves...

Revision rhinoplasty for bridge and tip
Dec 13, 2015
I believe your issue is more about the shape of your tip cartilages than the width. Your lateral crura are somewhat malpositioned, meaning they course more upward towards your eyes than outwards and this creates fullness in your supra tip. It also creates a sort of tear drop shape to your tip. Your alar rims are slightly retracted exposing more of your nostrils than would be ideal and both of these issues probably arose from your prior surgery and cartilage that was removed at that time....
Revision rhinoplasty for uplifted tip?
Counter rotating a short nose (or 'turning the tip downwards)
Dec 7, 2015
Dealing with an overly rotated tip or short nose is quite common in my practice and there are several ways in which to handle such situations. When deciding upon the method whereby the tip will be brought back into a more balanced position it's important to understand what caused the problem in the first place, as well as to determine precisely how the patient would like the nose to look at the end of the surgery. Sometimes the tip can merely be repositioned relative to the septum without...
Pinched nose, crooked tip?
PInched and crooked tip
Dec 4, 2015
Your tip is definitely twisted but thee may be complicated anatomic reasons for that which only your surgeon would understand at this point. If there are no significant structural risks I would think another operation would be in order to correct things. Five months is too early for surgery as things will continue to change and the entire spectrum of what needs to be dealt with has yet to be revealed. I would advise consulting with your surgeon soon but not actually doing any surgery for...

Disappointing consultation
Nov 14, 2015
One of the main reasons to consult with a few different surgeons is to ensure that the two of you communicate well and that there is a feeling of trust. Just like in the dating world, you will not get on well with everyone and certain personalties clash. Additionally, consultations allow the patient to get beyond the hype and see for themselves the surgeon, their staff and office as well as their demeanor. Consider it a worthwhile consultation if you have determined not to use...
Rhinoplasty and chin argumentation?
Rhinoplasty with improvements in the jawline
Nov 12, 2015
These two photos are rather limited for me to make a detailed and fully accurate determination. However, it appears that you are an excellent candidate for rhinoplasty as well as for modifying your neck/jawline. Your nose is over projected and this is exacerbated by your under projected chin. Although I haven't seen a frontal view, my guess is that your mandible is not just under projected but also narrow horizontally which makes your cheeks look more full. There are numerous sizes and...
Solution for alar rim dent?
Alar dent
Nov 6, 2015
Without photos it is difficult to respond accurately, however most depressions or 'dents' around the tip can be corrected by placing a graft in the area. These grafts can be cartilage harvested from the septum, ear or chest wall; or they can be soft tissue. The determination of what would be best to use should be made by your surgeon and often times these can be placed via a closed approach in a minor procedure.
Is this Polly beak deformity?
Pollybeak deformity
Nov 6, 2015
Currently the height of your nasal dorsum at the anterior septal angle is too high and this creates a 'hook' of sorts which is what pollybeak is referring to. Reasons for this can include a tip that is not projected enough, fibrous tissue forming under the skin above the tip creating a thicker layer, or to much cartilage being left in that area at the surgery. Sometimes it's the septum and sometimes it's due to the orientation and thickness of the lateral crurae that creates the fullness. It...
