Since you have had extensive weight loss I'd recommend a facelift and a fat transfer, this will help restore your mid face volume loss. Since volume loss is a part of the aging process, it's not uncommon for a fat transfer to be included during a face lift. Although a fat transfer does volumize, a lifting effect happens when fat is properly placed.
