The current lifetime risk for the development of ALCL is quoted as anywhere from 1/3,800-1/ 30,000, for women with textured implants. Common symptoms include breast enlargement, pain, asymmetry or a large fluid collection typically lasting more than a year. ALCL patients seem to develop an allergic reaction to the texturing around the implants. Therefore it is recommended that the implant and capsule be completely removed.

Breast implants can be removed through the armpit. It is easier to remove saline implants since they can simply be deflated and then taken out. Removing silicone implants is more difficult as they may rupture while being removed. If I do remove them through the armpit I will often use the endoscope to be certain no silicone has been left behind. If the silicone implants are all ready ruptured and they have a significant capsule around them, I suggest having them removed from an incision...

It appears from the photo's that the problem was corrected but you still have some remaining skin that did not contract around the implant. I have found that lateral displacement of implants usually requires a tightening of the breast pocket. This can sometimes be done with sutures if the breast tissue is not to thinned out and the implant is not too large. More often I have found that a dermal matrix such as Strattice or Surgimend is necessary to create the internal bra and hold the implant in place.

The risk of deep venous thrombosis or Pulmonary embolus is certainly higher in certain patients. Patients with incompetent venous systems are at a higher risk.
