There really isn’t an age limit to facial rejuvenation surgery. If you see things that are distracting to you when you look in the mirror or see yourself in photos, and those things are amenable to improvement surgically, then whether you’re 40, 50 or 70+ doesn’t really matter. This is a repairable problem, but the techniques required are techniccally challenging and it may require more than one surgery to achieve the results that you’re looking for. My recommendation is to seek out a consultation with a board-certified facial plastic surgeon who has extensive experience in revision rhinoplasty and septal perforation repair to understand fully what your options are.

Lola1406, thanks for your question. That is a regrettable outcome and the pixie ears and hypertrophic scars are likely due to excessive tension on the skin. My sense it that the scars will have to be surgically revised to give you the outcome that you’re looking for. That could likely be done under local anesthesia in the office.

Based on the length of your philtrum (the skin between your upper lip and nose) with your mouth closed I want to say yes, but the pic that really matters is how much of your upper teeth show when you have when your lips are slightly parted. The difference between an open and a closed rhinoplasty is the approach. Most facial plastic and general plastic surgeons can perform either approach, and the decision is informed by what type of reshaping maneuvers we’re trying to accomplish. Additionally, the open approach is preferred in patients who have had nasal trauma or prior nasal surgery (where we anticipate an increase in the technical difficulty of the procedure).
