Hello,
These do not appear to be wrinkles but normal contour transitions formed by the muscles around the mouth. Filler can help soften the appearance of these areas by filling them in, but care must be taken by the injector so that filler does not migrate beyond these areas.

It is still quite early for the Botox – Please wait at least another week to see if results set in. There are two possibilities here regarding the lack of response – Either you’ve developed true resistance to Botox and/or Dysport (which is extremely rare) or you’re being underdosed. Filler is temporary, and in the tear trough region, can last up to 12-16 months. Once the results disappear, reinjection is necessary. Short-term side effects include bruising and swelling, which resolve in days. Long-term adverse effects are few if performed correctly by the provider.

Hello,
Your perioral/cheek rhytids are not that deep, so I would recommend starting with a hyaluronic acid filler, such as Juvederm Ultra or Ultra Plus XC, may even one of the RHA fillers, which are better for treating dynamic lines and wrinkles. Radiesse is longer lasting and does not have a quick reversal if you want to dissolve it.

Yes, filler can definitely be used here. The hyaluronic acid fillers all last around the same time, 12-16 months. There are longer lasting fillers such as Radiesse, but this is not frequently injected into the oral commissure/perioral region.

It is very hard to tell the underlying cause without direct examination and, even then, without directly lifting the skin up to assess the tip cartilage underneath. If your nose became more bulbous over time, then this may be due to the way the nose healed/scarring. As you have determined correctly, there are many factors at play here. If we drop a straight line down from the deepest part of your forehead where it meets the start of the nose, your chin is underdeveloped (the lips should protrude a few mm past this line in cases where the chin is adequately developed).
