Thank you for your questions. It is difficult to fully assess your nose as multiple views are needed in order to create the best plan. I perform closed and open a Rhinoplasty and there are key indications for each. Schedule a consultation with a board-certified plastic surgeon experience in rhinoplasty and he or she can create a 3-D simulation to show you what can be accomplished through either method. Remember, the nose is a 3-dimensional structure and must be approached as such. Thank you for your question. A lower radix (space between the eyes) can often make the eyes appear farther apart. There are two solutions, one temporary and one permanent. Adding hyaluronic acid filler to "build up" the radix and bridge of the nose can often improve the appearance and is a good temporary fix. Undergoing a formal rhinoplasty is another option to permanently build the dorsal/radix height.

After reviewing your photos, I do think that piezoelectric (ultrasound) shaping of your nasal bones is an effective option.  I use the piezoelectric instrument during my rhinoplasties to reduce bruising and swelling while appropriately reshaping the upper 1/3 of the nose.

This is an easy answer for any rhinoplasty surgeon.  An easy and effective tool for raising the height of the radix with/without other methods for reshaping the nose is to place cartilage grafts.  Stick with simple and you will have the result you are looking for.I recommend meeting with a Board-Certified plastic surgeon experienced in rhinoplasty.  He/she will effectively examine you, create a 3D simulation and devise a treatment plan tailored to your needs.

Alar base resections can often improve the frontal aspect of the nose, but rarely effects the profile view.  I recommend an in-office consultation with a Board-Certified Plastic Surgeon who will thoroughly examine you, create a 3D simulation, and develop a plan that will tailor your nostrils +/- nose into a more aesthetic and refined version of their existing structure.

It appears from the photo that you have very oily skin.  Oil contains bacteria, and it is likely that one of the deeper stitches in the alar base is infected.  Ultimately your surgeon is correct:  I would also recommend antibiotic ointment and it may be that the stitch may "necessitate" its way to the surface.

Without a frontal or worm's eye (underneath looking up), it is a little difficult to tell whether or not you will require alar base resections.  With every procedure, I inform my patients that they make require this to create the perfect tailoring that is usually performed at the end of the surgery.  If your surgeon states that you will likely not require alar base narrowing, then believe him/her.  You are trusting this person to make the correct decision based on their knowledge and expertise.

Thank you for your question.  This appears to be a full-thickness injury which is a very rare complication.  I believe that the best course of action is to undergo microneedling vs Fractora to improved the topography (the depression) of the scar.  After the texture has been improved, then I would laser the pigmentation to match the surrounding skin.
