In order to significantly improve your jowls a facelift is the procedure of choice. Fillers can not lift and reposition the tissues. You may also consider buccal fat removal. Consult an experienced facelift surgeon and discuss your options in detail. Good luck.

Yes, you can have a rhinoplasty and not take steroids. Steroids help reduce swelling more quickly, but there is no need to take them for a rhinoplasty. Discuss this with your surgeon, but it should not be an issue. Good luck.

You will have a much better result from your nasal surgery if your dorsal hump is removed and your bridge narrowed. These changes can easily be performed, in addition to refining your tip, under IV sedation using a closed technique. Consult an experienced rhinoplasty surgeon and address these issues. Good luck.

It appears that you would have a good result from a rhinoplasty. Your dorsal hump can be removed, your bridge narrowed, and your tip refined. It is true that thicker skin does limit some of the narrowing that can be achieved in the tip, but you should still be able to have a good result. When a dorsal hump is removed it is necessary to move the nasal bones closer together to prevent a wide dorsal area (“open roof”) deformity. Without seeing photos it is difficult to specifically comment on your surgery. However, this can be corrected in a secondary procedure if this is what has happened in your situation.

The short answer is that you can easily do them at the same time. A subnasal lip lift is performed by making an incision around (and below) your nostrils. This would not interfere with a lower facelift at all and you would recuperate from both at the same time. Good luck.
