There are no hard and fast rules for breast reduction but a DD on a 40 bust is a heavy breast but not such that will make a mastopexy or reduction easy or safe. There are techniques using a ‘central mound’ pedicle which are relatively safe and you should consider these, however, the caveat is that a C cup cannot be guaranteed
