It is in fact difficult to correct as the initial cause is related to an over-pulling of skin in an unnatural vector instead of focusing on the underlying tissue support without "pulling" the skin. It is difficult to recommend other options without some images-non smiling.
It may be possible to reduce some of the appearance with filler or possibly "neo threads".

At 4 weeks I would expect an overall better appearance. It appears that the incision is quite red and reactive. There may also be some (probably temporary) hair loss. You should consult with your surgeon regarding this. There are a number of options to promote better healing such as addressing any potential inflamatory reaction as well as treatment with "V-beam" therapy.

Ultimately, the timing is an individual choice. Your images show some degree of platysma banding and laxity which is in fact very correctable with potentially even better results than your prior lift. If it is reasonable for you I would encourage revision which should then result in significant longevity.

The answer is actually somewhat dependent on the individual. We tend to add fatty tissue in different patterns and if the face in in fact one of the primary areas of fat deposition in an individual then it does makes sense to achieve the reasonable weight goal prior to your procedure. If, on the other hand, one is predominantly adding weight to the lower body with limited affect on the face and neck then it really doesn't make much of a difference.
