For improvement of your side profile, I would like to suggest chin filler for a forward projection. Your Nasolabial grooves seems to be quite deep, and I would like to do cheek fillers to lift up the center mid cheeks. I can see your tear trough is sunken, so the cheek fillers can improve that. You also seem to have volume wasting in your temporal area, I would use fillers to restore the volume lost there. Your forehead contours can also be improved with fillers.

I would suggest that you stop triluma immediately. There are some people who reacts badly to hydroquinone. Treatment for melasma is multimodal approach, oral medication combined with topicals such as retinols and laser would be the best approach to controlling melasma.
