Yes, a full vaginal reconstruction (consisting of deep vaginoplasty + perineoplasty) can be performed under awake, purely "local" anesthesia (different from "conscious sedation" which basically is "...mostly asleep..."). First designed by Dr. Red Alinsod around 2010 and adopted by myself (Dr. Michael Goodman) the same year, for many years only Drs Alinsod and Goodman performed these vaginal tightening surgeries on awake patients, under "local." I can appreciate your distress over the horrific results of your previous labial reduction. Unfortunately, I have seen this result several times. Each time the initial procedure was performed by a general Ob-Gyn who simply was not trained properly to do the job right, resulting in the appearance you have experienced. Additionally, I have seen this outcome in women who smoke, resulting in the fistulas (the "tunnels & openings...) This is what happens with wrong sutures... wrong technique. ...

No one "needs" a clitoral hood reduction, but if you elect to undergo labial reduction surgery, you may wish a hood reduction as well for better aesthetics/ "flow..." Unfortunately, your one photo does not show your hood at all, so I cannot answer further. If you wish, you can submit additional, IN-FOCUS pics that additionally show your hood.

I agree with Dr. Reed, my former trainee. It is not rare that when performing a labiaplasty, especially via a "linear/trim" technique, that the surgeon is not sure how to handle the lowermost portion of the labia, especially if the redundancy involves the area at the posterior vaginal opening. Your revision, if performed by an experienced surgeon savvy in working around the posterior commissure, should well accomplish what you wish. I see you live in Phoenix. Aahhh... The "hype" of Labial Puffing. Same as the hype of all of the "minimally invasive"/ transfer/ "non-invasive approaches. You have to keep coming back! Every 6 months, every year, every couple of years. Practitioners should accurately inform their patients as to length of effectiveness, and many do. But in my opinion, most do not. Women (& men) are used to "coming back" for therapies (look at Botox!). Labial puffing is not permanent. If nanofat + PRP is utilized, perhaps the results...
