You look like you have been taking care of your skin-I can't see the sun damage you refer to, so--keep it up. Use good products! Make sure you also prevent deepening wrinkles with regular Botox sessions--they work!You would benefit from temple, nasolabial fold, and maybe a little (natural) lip filler placement to add more youthful looking volume. I always recommend starting with baby steps-Botox and fillers are easy, low risk way to maintain (or achieve) a more youthful appearance.

I recommend (and do) buccal fat pad reduction with submental liposuction often. It makes since as you will already be in the procedure room and the areas will recover at the same time. I often will do these with a chin implant at the same time.Go for it!

Filler, Fat, or Renuva

You certainly can have fat grafting to replace the asymmetrically removed fat. Yes, it may take more than one session as not all the fat transferred survives the transfer process. Another option is a new product called Renuva which is an injectable composed of adipose allograft matrix with collagen, proteins, and growth factors derived from donated and purified fat. This stimulates your body's stem cells to make more of your own fat. A bit pricey, but--no surgery.
Please advise if I should consider buccal fat removal or an alternative procedure?

Typically, buccal fat pad reduction is recommended in younger persons. As we age we lose volume in our faces. I tell all my buccal fat pad reduction patients that it will look good in your 20s and 30s, and you will probably benefit from strategic fat transfer back into areas of the face in your 40s and 50s+.It isn't a big deal for you from a surgical standpoint-short, minimal pain, risk, recovery- but you might "miss" the fat in a few years.
Acne scars seem deeper after TCA CROSS. How can I treat them?

I have found TCA CROSS helpful when it is done several times over several months. I would recommend a few more sessions with 70-100% TCA. Best outcomes with acne scars is usually achieved when multiple modalities are combined.

I would recommend starting with a dermal filler (such as Bellafill) placed at the same time as subcision under the scars is done. This may be repeated 2-3 times separated by 3-4 months. In addition, TCA CROSS (trichloroacetic acid chemical restructuring of skin scars) done monthly for 3-4 sessions can help even out the scars. Surgery is a big deal. I would certainly recommend you start with a "Botox browlift" -which is a real thing. Its easy, inexpensive (you can do it for years for the cost of a surgical browlift), and if after "test driving" the lifted effect, you can proceed with a tuck or a browlift.

Placing filler (many various types can be used) strategically under the tail of the eyebrow and over the orbital rim lifts the brow in a subtle, beautiful way. What you ask for is certainly possible...but be careful what you ask for. A "cute, button" nose can compromise your airway. A cute nose you can't breathe through is a problem you will NOT tolerate for long. Be sure your surgeon has a balanced approach between form and function!
Is Buccal Fat removal right for me?

You are highlighting jowling. Buccal fat pads are in the central cheek, and reducing them will not help jowling significantly. Your options are to camouflage with filler placed along the jawline in front, and behind the jowl, or to simply under go micro-liposuction. Godspeed!

Two general schools of thought with facelifting: 1) Do it younger, which slows and softens the facial aging process, but results are more subtle--no "wow"-- as less is required. 2) Do it older, enjoy much more of a dramatic "wow", look a lot younger--but forego the anti-aging benefit through middle age.
Dr. D. Antell published a fascinating study with identical twins that showed that facelifting provides a permanent benefit that I think does argue for doing it younger.

The bulges you are seeing are due to how your facial muscles are shaped and function. I do think overall your chin shape looks just fine, however you could "soften" these areas by strategic placement of a little Botox and/or hyaluronic acid filler. Conservative amounts are the rule here though, as too much can cause your smile to look different!

The microspheres of Bellafill are so tiny, and completely surrounded by your own collagen that there is literally no way enough heat can be applied to them to melt/change their properties without complete destruction of the tissues the Bellafill is in. I have performed laser skin resurfacing using CO2 lasers -which is hot enough to vaporize the water in skin -too many times to count and I have never seen an effect on the underlying Bellafill. I have seen problems with temporary inflammation...
