At 4 weeks post-op you seem to be healing well. It is normal to feel some tightness and swelling around the sutures. I do not see any significant wound separation to worry about. I predict you will have a very good result! Best wishes!

An in person consultation is necessary for a thorough evaluation, but I agree that surgery (neck/face lift) is the best and most effective option. For someone of your size, 2 liters of liposuction aspirate is a significant amount and should show a difference. As you may know, liposuction is an art and a science, and the results are more dependent on the surgeon's skill and expertise than the total volume. Your age is not a contra-indication to consider fillers. Hyaluronic Acid fillers are relatively safe, effective, and not permanent. In the right hands, you will see an improvement that is natural looking. Upon direct examination, your plastic surgeon can assess your abdominal wall muscles and the benefit of the muscle tightening that can be done. We frequently contour the whole area with simultaneous liposuction and get further improvement of the results. You can certainly get the shape improvement that you want with surgery.
