Answer Snippet
Asymmetrical eyes/eyelids - is there any way I can fix them?
Assuming you have not flip your photo prior to posting, you have left upper eyelid ptosis.
Nov 23, 2023
However, you also have a crease problem. This is likely ptosis caused by central levator disinsertion. You have minimal left compensatory brow elevation. You need a detailed oculoplastic surgery consultation to determine if there is levator pathology also on the right side. Herring's law ptosis would be a fall in the upper eyelid position of the other eyelid after fixing the obviously heavy eyelid. In addition to the repair of the ptosis, you also need your crease fixed. I strongly suspect...
7 months and face is still too narrow from masseter Botox
Be patient.
Nov 23, 2023
The masseter volume will come back. In the mean time, I recommend removing your migrated filler. This will likely help the brain process the face. Bad volume has a way of messing with your perception of the face. That is something you have control over right now. The masseter volume may take a year to recover.
How soon after a cholecystectomy can I have a lower blepharoplasty done?
Do not even think about doing this at the same time.
Nov 23, 2023
There is absolutely only one person on earth who can answer this question and that is the general surgeon who will be doing your gallbladder surgery. The vast majority of these surgeries are performed endoscopically with or without robotic assistance. Recovery is much faster than open gallbladder surgery. Still we are talking about abdominal surgery. Hopefully your surgery is uneventful and you are quickly on the mend. Your general surgeon will give you an idea of when they think it is...
Filler injected superficially resulting in bumps - should I go back to the injector?
You don’t need ultrasound to see bumps.
Nov 22, 2023
Yes get these fixed. Why didn’t the second office offer to fix this? Don’t live with bad hyaluronic acid filler. Get it fix and find a different injector for next time.
Are these festoons? How to treat them?
This is very likely your ancient filler.
Nov 20, 2023
These fillers likely last forever in some form. As they age, they hold water in a way they did not when they were first placed. I recommend having a significant hyaluronidase treatment. One week later you can have a restorative filler service. For my traveling patients we often do the enzyme service at the beginning of the week with the initial consultation and the filler service at the end of the week before they leave the area.
Which procedures would help me achieve my desired eye shape?
I have viewed this post and your previous post.
Nov 18, 2023
Inevitably you need a very detailed, in-person consultation to define precisely what you need in a reconstructive surgery. This cannot be Zoomed. Cosmetic orbital decompression is associated with a host of post-surgical changes that are often not well appreciated even by surgeons. The same can be said for blepharoplasty. You are left with both lower eyelid retraction and what I have described as post-upper blepharoplasty syndrome (PUBS). Lets talk about the upper eyelid findings first. I do...
Uneven upper eyelid crease after skin pinch, what would you suggest?
Do not let anyone fix this by removing skin from the more normal side with the lower crease.
Nov 14, 2023
Surgeons are accustom to looking at pictures with you in the anatomic position. Commonly people confusingly reverse their photos to reflect what they look like in the mirror. I am going to discuss your eyelids on the assumption that you have not flipped the image and the eyelid with the high crease is your anatomic right eyelid and the more normal appearing eyelid is your left eyelid. The problem here is that there really is not such thing as "skin pinch" upper eyelid surgery. That does not...
Bulges under eyes post Botox, is this fluid or fat herniation (something else)?
The lower eyelid is not really a good location for Botox.
Nov 14, 2023
This is not swelling. This is weakened lower eyelid muscle. Your muscle is paralyzed and slumped causing the tissue to heap in this unattractive fashion. I recommend not chasing this problem. Eventually the Botox will wear off and hopefully this issue will resolve. Specially, do not well meaning providers laser or inject this, you will end up with a permanent and hard to fix problem. This can take 4 to 6 months to resolve. Also find a new injector,
Which procedure would help correct my asymmetrical eyes?
You have a small facial asymmetry and bilateral upper eyelid ptosis.
Nov 14, 2023
Generally it is impossible to cosmetically adjust the relative heights of the two globes nor is it even necessary. This type of asymmetry is very common and generally it does not bother most people nor noticed by others. While there are surgeries like floor implants or even more radical surgeries that could adjust the relative heights of the eyes, the complications associated with these procedures are too common and severe to warrant these interventions except in cases where the physicial...
Uneven lower eyelids after blepharoplasty, advice?
Whoever did this surgery really does not understand eyes.
Nov 13, 2023
I am not saying that they don't understand eyelid surgery. Aesthetics are all about balance. Performing subtractive upper eyelid surgery seldom achieves balance. You have partial central levator disinsertion, with compensatory eyebrow elevation. Your surgeon ingored the pathology of the central levator aponeurosis. I call your post-surgical situation Post Upper Blepharoplasty Syndrome. It is possible to repair the upper eyelid tendons, lowering the upper eyelid creases, even restoring some...
Should I revise this upper bleph/droopy part on my left eyelid?
I can't advise you to have this surgeon do this revision.

They created the issue. What is the likelihood that they can fix the issue without causing a further issue? The answer is that it is impossible to know. It might be great or it might cause a new, unfixable issue. The removal of a small amount of skin in this focal area under the slight area of fullness would likely fix this. May advise is to not take the risk. Yes it is understandable that it bothers you but overall the results is very good. Sometimes the best way to get out of trouble is to...
Age 42, strange mound of skin/fat, not interested in fillers as a fix
A facelift would destroy you. Please do not do that.
Nov 10, 2023
It is a same that you are not open to fillers because that is the state of the art for addressing issues like this. A small amount of filler just below this area of fullness would help it blend. Many of us benefit from this type of service. Please understand that not all filler services are the same. You are asking for your face to be sculpted. They do not teach that in Nursing school, PA school, or even medical school. Caution is advise.
Could I fix my deep set/hooded eyes with an upper eyelid surgery?
You do not need eyelid surgery.
Nov 10, 2023
That means that you could have a very fussy upper eyelid surgery to expose a small amount of your upper eyelid platform to create a small bit of increased eyelid platform show. However, most eyelid surgeons are not restrained. They will aggressively remove your upper eyelid fold. This will skeletonize your upper eyelid making you look prematurely older. You will hate the result. Extreme caution is advised. In my experience only 0.01% of eyelid surgeons are capable of delivering what you need...
Eyelid asymmetry - do I need ptosis correct, blepharoplasty or filler to treat volume loss? (
What you need is going to be very hard to find.
Nov 10, 2023
You need a very fussy crease lowering surgery that entails a very precise ptosis correction, crease lowering surgery, and advancement of the preaponeurotic fat. This is aesthetic upper eyelid surgery at a level of precision only practiced by 0.01% of eyelid surgeons in my experience, which is extensive. That will make it very challenging to get this work where you live. Your eyes can be your best feature.
Are there alternatives to hyaluronidase that can be used to remove lip filler?
So sorry you have had a bad filler treatment.
Nov 10, 2023
Hyaluronidase, the enzyme that breaks down hyaluronic acid fillers like Restylane. I am not sure I would follow the advice of a doctor whose license is on probation. It is not the case that fillers just disappear. This volume problem you have could last for years. Do not be afraid of the enzyme. There are several lay people who post scary stories about enzyme. Mostly these people have had bad filler service and then poorly under-dosed enzyme treatment. This can cause mobilization of...
