With minor surgery you could probably obtain some good improvement. The least involved would be a submentoplasty which could be done under local anesthesia and an incision in a neck crease beneath the chin. The skin could be smoothed and some excess skin removed. It is difficult to determine the cause for sure without an examination. In certain people and attached frenulum which is tight can cause this. This would feel like a tight band running vertically in the midline of your upper lip. If this were present it is very simple to fix and perhaps to solve your problem. Some small amount of Botox could also provide a temporary solution.

It is difficult to analyze from this photo, but it is probably scarring from acne or some trauma. This could be treated with laser resurfacing. In our practice we would use the Sciton Contour or Pro fractional laser. Radiofrequency microneedling would also be beneficial. We recommend SyldfirmX. These treatments are done under topical or local anesthesia.

Can I do rhino, lip lift & lower blepharoplasty all in one?

It is wise to combine procedures as possible. It saves on anesthesia and typically recovery times are in parallel- in other words if a lower blepharoplasty and rhinoplasty and lip lift each have a one week recovery the recovery for all 3 is not much more. Some procedures shouldn't be combined but these 3 can be.

You are young but seem at least in this photo to have excess fatty tissue in your lower lids. An in-person consultation could confirm this. If there were excess fatty tissue it could be removed with a small incision inside the eyelid. This would leave no external scars and is low risk. In your case this is likely not due to aging but just a familial trait. It is important to not be too aggressive and create a hollow look.

You seem to be a good candidate for a transconjunctival lower lid blepharoplasty. A small incision is made inside the lid and the excess fat removed or repositioned. You likely wouldn't need any skin removal, but various nonsurgical procedures could be used if there were skin laxity present. An in person consultation would help define a specific plan.

We have a long experience doing both CO2 laser and Erbium laser combined with facelift. Assuming you are careful with the settings over the flaps it is safe. We have performed thousands of the these cases. The most common is to combine resurfacing around the mouth and eyes with a facelift which is completely safe. The only issue with the combination is to be careful with any dressings so as not to damage the healing skin.

The "11's" and forehead lines are best addressed with a neurotoxin such as Botox or Dysport. The marionette lines are best addressed with a hyaluronic acid filler.
