Statement: Born and raised in Houston, Dr. Farhood’s grew up with parents were also practicing physicians. They would frequently bring him to work marking this early exposure to medicine as what piqued his interest into the field. In high school, he sustained a wrestling injury and was treated by a facial plastic surgeon who was also a former wrestler. The surgeon later came to one of his matches and left an indelible impression on him regarding the impact medicine can have outside of the operating room.

Dr. Farhood graduated Magna Cum Laude from the University of Texas, Austin with a Bachelor of Arts in Biology and completed his medical education at the University of Texas Health Science Center in Houston – home to the world’s largest medical center. In his final year, he was inducted into the prestigious Alpha Omega Alpha honor society.
After obtaining his medical degree, he completed a clinical research fellowship in the Department of Otolaryngology at the Medical University of South Carolina where he conducted numerous clinical trials, published extensively in the scientific literature, and presented his work at several national meetings. He has produced over 50 peer-reviewed publications, presentations, and book chapters. He then completed a five-year residency in Otolaryngology-Head & Neck Surgery at Saint Louis University, earning multiple accolades for his teaching, compassionate care, and scholastic achievements.

Following this, he pursued advanced fellowship training via the American Academy of Facial Plastic & Reconstructive Surgery at the University of Toronto, ranked by Times Higher Education as 5th in the world for clinical and health and 2nd among North American public institutions. The important thing to keep in mind is that when performing rhinoplasty on thick-skinned patients, the nose cannot be overly shortened or reduced- otherwise it will loose shape and definition. I typically recommend maintaining tip projection in these patients to keep the skin well-fitted around the underlying nasal structures. I think the edited photo you made is a reasonable goal given that you did not overly deproject your nose.

It’s hard to give a complete answer given the limitations of your phot but yes, a rhinoplasty would help improve the convexity of your tip cartilages. Essentially they would be reshaped to give your tip a less rounded appearance and improve its definition. Schedule a formal consultation with a facial plastic surgeon to discuss your goals.

Combining ptosis with an upper blepharoplasty is absolutely an option and easily performed as the incision is often the same. Usually they are combined in older individuals with excess skin, it would be unusual to perform a blepharoplasty in a young patient without dermatochalasis. Any nasal surgery has the chance to impact olfaction (smell) postoperatively. Most often this is related to swelling and should resolve over a month. It is unlikely for smell to be permanently lost after rhinoplasty unless other procedures were performed with it such as endoscopic sinus surgery.

This skin type can be the most challenging for surgeons and patients. I would discuss with your surgeon and aesthetician about an appropriate regimen. Some surgeons recommend accutane postoperatively to help with the healing process as this can shrink the sebaceous glands and assist the skin in contracting over the newly designed nasal shape.

Radix height is best evaluated from the profile view; not the frontal view. In men, the radix ideally sits at or above the lid crease. 5 1/2 weeks is quite early to judge the result. Whenever the radix is altered it tends to hold swelling longer.
