Dental surgery is probably the most common cause of delayed filler reactions/infection in my experience. Probably what happens is bacteria enter the blood stream during the dental procedure and then hide in the filler like a biofilm. I would recommend melting ALL the old filler before you inject more so as to make sure there is no biofilm left. These delayed reactions are rare so you should be fine if you avoid more dental procedures.

The reason you often see the angle raise with deprojection is that a common technique to achieve de-projection is lateral crural overlay where the lateral crura are cut and overlapped…this causes increased rotation (tip up) as it deprojects. You can use tip sutures to minimize the rotation caused by lateral crural overlay, or perform a more intermediate crural overlap where the cut is closer to the dome (tip). Find a good, experienced rhinoplasty surgeon and you should be fine.

Hi Nana: It probably is from allergies and pushing up on the nose as you were growing. You could try injecting a filler like vollure which should fill the crease and make it about 50% better. This is temporary, but also reversible, which is nice. Over time, you could probably achieve 80% reduction of the crease if you are no longer pushing up on your nose and you keep up with the filler.

Hi Jackie: Rim grafts are very safe and very effective. There is typically no real “incision” made, just a pocket dissected along the nostril margin. Rim grafts help maintain a normal nostril shape and normal nostril symmetry over time after rhinoplasty, and can help breathing as well.

I would start with botox. Basically with Bells palsy, the facial nerve gets weak and then when it recovers, it sometimes recovers too much and you get tightness and extra movement, called synkinesis.

I have about 30 patients like yourself that I see every 3 months for botox treatments. It can usually open the eye up, and often insurance pays for it. In your case, if the eye is the only complaint, it would be pretty inexpensive even if insurance didn’t cover it as you would only need…

I think you should do both. You could “get away” with fillers in the chin, but the cost will be much more over time doing fillers and fillers probably won’t give as good a result as a chin implant would. Most surgeons will decrease the price of chin implant placement if done at the same time as the rhinoplasty.

Yes it is possible to take down the dorsal hump without altering your tip. However, taking down the bump does a few things. Firstly, it makes your nose look wider, so you would need your nasal bones broken to re-establish a normal width of the dorsum. Also, if you take a bump down, it has the illusion of making your tip look like it projects much further from the face on side view. Hi I agree your nose at rest has a pretty good appearance. You have really thin skin which is why you can see so much contour of your lower lateral cartilages (tip cartilages) and also the asymmetry. Other areas for improvement would be taking down the dorsal hump conservatively, and supporting the tip so that it would not pull down so much when you smile. Finally, on front view your tip has quite a bit of bulk and roundness, which could be reduced with conservative cephalic trim and tip…

The “bags” I think you are talking about are folds of the orbicularis oculi muscle, which is a circular muscle around your eye that closed the eye. In some people, the muscle is stronger or more folded, making it bunch up a little more. Having this is actually considered youthful, and people in Asia actually get filler to CREATE this bulge.

That said, I agree yours are quite prominent. You might try a very small amount of botox into the area, like 2 units per side, to see if relaxing…

The darkness is partly because you have nice strong brow bones and beautiful big eyes, and those 2 things shadow the under eye area from overhead lights, which is almost all the light in our world (sun, ceiling lights, etc.).

You appear to have developed a more visible “tear trough” which is the hollow under the eye closer to the nose where muscles attach to the eyesocket (orbital rim) and pull the skin inwards. It does tend to develop in people as they enter their 30s and particularly in people whose cheekbones may not project forwards as much, as well as people who are more thin.

Hi: Thanks for your question. First off, you have a pretty face. Your nose certainly is still ethnic appearing, but that would typically be my preference and may be your preference too. I don’t see facial asymmetry that is enough to be distracting…most people have a little bit of facial asymmetry. You have a stronger nose on profile but also a strong chin, so they balance eachother.

Hi Lanm: Your goals seem achievable. Tip/nostril asymmetry in someone who has never had surgery is due to either a deviated septum, or asymmetry of the tip cartilages, or both – and these things should be able to be significantly improved by a competent rhinoplasty surgeon. You probably have baseline nasal congestion that is worsened by allergies and then you got addicted to the nasal drops. I would recommend the following:
1. wean off nasal drops, even at night, by switching to a nasal steroid spray like Flonase
2. have a full allergy evaluation and consider allergy shots or at least an aggressive allergy regimen like flonase, zyrtec, saline sprays and possibly an additional medicine like singulair
3. Have a full ENT evaluation as you could have a severe…

I would start with trying filler injection as it is quick and quite safe, and can be reversed (melted) if you don’t like it. Surgical revision is possible with repeat osteotomies and spreader grafts and/or nasal stents to keep the nasal bones more lateralized, but this is higher risk and I bet fillers can get you the result you want.
