Will there be any long term effects if I drink alcohol after rhinoplasty?
Alcohol consumption after rhinoplasty
Oct 2, 2023
In my practice, I advise patient to avoid alcohol for two weeks after rhinoplasty. We caution patients that during the first year, there may be some triggers that might increase redness and swelling. Alcohol is a common trigger. Spicy foods, exercise and temperature changes may also have a similar effect.
Skin on nose after rhinoplasty. Is this normal?
Skin changes after rhinoplasty
Oct 2, 2023
Thank you for sharing your question and postop rhinoplasty photo. The photo provided is a little blurred. So, I will make some general comments and hope this information is helpful. Infection, skin necrosis or other serious complication is extremely rare outside of the first few weeks postop. Hyperpigmentation is more likely. Sun exposure, especially for patients with darker skin and those prone to hyperpigmentation (by history) may cause pigment deposition in the skin following rhinoplasty....
Was the DP facelift & N/L done properly?
What needs to be done to correct the neck issues?
Sep 26, 2023
Thank you for sharing. You still have some fullness in the submandibular area that is likely due to the submandibular glands and medial platysmal banding. While you may have had a deep plane facelift, you've had too conservative of an operation in the neck. Deep neck surgery involving pulling the platysma muscles together and possibly reducing the size of the submandibular glands (versus additional tightening of the platysma muscle behind the jawline) would improve the concerns. In short,...

Options for treatment: deep plane facelift?
Sep 26, 2023
Thank you for sharing your photos and questions. You have a mild of aging in the neck and jawline and moderate in the midface. There is elongation of the upper lip. A true extended deep plane facelift (with complete mobility of the midface and malar fat pads) combined with upper lip lift would address the concerns satisfactorily. There is plenty of midface volume so that a fat transplantation is not necessary.

Hypertrophic scarring after facelift
Sep 26, 2023
Thank you for sharing your questions and photographs. You have some hypertrophic scarring behind the ears that may be a result of either increased tension on the skin or simply a consequence of how your tissues are healing. In general, a small amount of hypertrophic scarring can be treated with some dilute steroid injection and, if treated promptly, will likely flatten out over time. Ultimately, this type of scarring can be revised once the healing process has completed and, fortunately,...
Facelift after tightening procedures?
Facelift after non-surgical facial rejuvenation
Sep 26, 2023
In my practice, the majority of all facelift and deep plane facelift patient have had prior treatment, including fillers, neuromodulators, ultrasound and other energy-based therapies. Many patients have had prior facelift surgery, as well. While it is true that some of these treatments may increase the difficulty of a facelift or neck lift, an experienced surgeon will be able to explain precisely what challenges and limitations may be a result of prior treatment. However, (excluding serious...
What is the best way to get rid of scars from my deep plane neck and facelift?
Treatment to improve deep plane facelift scars
Sep 26, 2023
Thank you for sharing your questions and photos. It appears you've had some skin flap necrosis near the hairline behind the ears. Generally, these still heal very well over time. Although, the wounds will need to be managed throughout the first year. Redness and inflammation in that area may indicate some hypertrophic scarring (or keloid formation). This generally responds well to steroid injections and/or light based therapy such as IPL or laser treatment. Be sure to continue to follow up...
Deep plane facelift or SMAS facelift for 60 year old?
Pros and cons of deep plane facelift and SMAS facelift
May 26, 2023
I suspect the main reason why 3 surgeons have recommended SMAS facelift while only 1 surgeon has recommended deep plane facelift is that the surgeons will recommend the procedures they are most comfortable performing. A much smaller fraction of facelift surgeons perform true deep plane facelift surgery. It wasn't until the last decade (2010's) that more modern techniques of deep plane facelift surgery, such as extended deep plane facelift, were described and it will be even longer before...
What can be done to improve my lumpy neck skin?
Central neck skin irregularities after necklift
May 19, 2023
The skin contour irregularities are unlikely to improve with time and nonsurgical treatments such as steroid injection. Unfortunately, this problem requires revision necklift and lower facelift to correct. Additional surgical correction is needed in the deeper neck tissues. You may need some reduction of the deep neck musculature (digastric muscles) and perhaps even some reduction (not removal) of the submandibular glands to completely correct these issues. These problems are the second most...
Loose skin under my chin and neck after facelift revision - can this be corrected?
Residual platysma bands after neck lift
May 19, 2023
Thank you for sharing your questions and photos. This is a very common problem. It looks like there are some mild platysma muscle banding. This can often be treated in the office under local anesthesia using an incision under the chin. It appears you may have already had some central neck surgery, perhaps in an attempt to improve these bands. However, You have some pleating centrally that may require addition skin elevation and repositioning as well as repositioning and suspension of the...
Facelift wound will not heal. What can I do?
Prolonged skin healing problems after deep plane facelift.
May 18, 2023
After 8 months I'd be concerned there is something else going on prohibiting wound healing. Other things to consider are retained foreign body (stitch), skin cancer, chronic low grade infection and salivary fistula/drainage. Biopsy, wound cultures or evaluation of any drainage for saliva proteins can be performed to investigate further. Additionally, if the wound is scabbing and crusting it is possible you may not be caring for the wound appropriately. Often, we use antibiotic ointment and a...
Will a lower facelift correct the loose skin under my chin, and lift the corners of my mouth
Will a lower facelift address the loose neck skin and corners of the mouth?
May 18, 2023
The short answer is a neck and lower facelift may not be enough. Even though you are young, the heaviness in the neck and lower face may require more advanced techniques, such as deep plane facelift and deep neck lift, to achieve a satisfactory result. The corners of the mouth are typically not lifting (at all) with traditional SMAS lower facelift and require an extended deep plane approach in which the entire midface and midfacial fat pads are mobilized and key retaining ligaments of the...

Asymmetry after deep plane facelift
May 18, 2023
Thank you for sharing your questions and photos. There is clearly asymmetry of your face. However, your photos and hairstyle limit a complete assessment. It's possible there was some existing asymmetry prior to surgery. Take a good look at your preoperative photos and compare. This could be an issue where the problem is asymmetric volume. In that case the most appropriate treatment may be volume replacement (fillers, fat transfer, etc.). In any event, I'd recommend waiting at least one year...
1 year post upper eyelid surgery - was enough skin removed?
Skin removal and brow position for periorbital esthetics.
May 18, 2023
Thank you for sharing your photo and questions. To get further improvement you would require an endoscopic forehead lift and (possibly/likely) addition skin removal from the upper eyelids. While the brow lift would be subtle, it is unlikely you'd get satisfactory results from upper blepharoplasty alone. Your primary surgeon was exercising good judgement in not offering additional surgery and being conservative.
Is a Facelift the best solution for festoons?
Festoons after deep plane facelift
May 18, 2023
You can expect swelling in the "festoon" area to last anywhere from 3-6 months after deep plane facelift surgery. It appears you may be within this time frame. How long after deep plane facelift were these photos taken? This 3-6 months time frame may be longer in some cases. I have noted increased edema / swelling in this area in patients who have had extensive filler (and malpositioned filler) in the cheek, lower eyelid, and tear trough area following deep plane facelift surgery.
