Since the spreader grafting is placed between the upper lateral cartilage and septum the upper lateral cartilages will be moved further away from the midline (laterally) after spreader grafting. Sutures are used to secure the newly positioned upper lateral cartilages to the spreader graft and septum so it won't move out of place.

It can be difficult to procure these outside records, especially if they're from many years ago.

You'll want to ensure that your revision rhinoplasty surgeon has extensive experience and can adapt the plan depending on what is found during surgery.

Yes, I can see how your profile could be refined with rhinoplasty surgery. This could create a less full bridge on side view. Besides your upper bridge bump you also have an area of fullness in your supra tip region (the area just above your tip) that could also be addressed at the same time.

Rhinoplasty can be done at the same time as turbinate reduction if you like.

Most general Ear Nose Throat doctors don't perform rhinoplasty. It is very common to have both cosmetic rhinoplasty done at the same time as nasal breathing improvement surgery like you're looking to do.
You could find a facial plastic surgeon who is board certified in both ENT and facial plastic surgery who would be able to treat both issues.

Yes, there are limitations as to how much bone narrowing can be done with osteotomies. I find that performing medial, lateral and intermediate osteotomies can help in maximizing how much narrowing is possible when someone has extreme baseline bone width.

We don't normally remove any nasal bridge skin, but plan on having it shrink wrap over the new underlying bone position.

Endoscopic septoplasty is not a common or standard approach to perform septum surgery. In my experience using an endoscope for septum surgery is not normally needed and doesn't reduce the side effects to the procedure.
The endoscope is a small telescope that gives the surgeon a magnified view inside the nose and is typically used with sinus surgery, but since the septum can be worked on with just a headlight and speculum we don't normally need an endoscope. In either case an incision would...

A standard septoplasty on its own wouldn't be expected to make your external nose straighter, so if that's a concern you'd likely need a rhinoplasty.

Without examining your nose it's hard for me to say what would need to be done to improve your congestion symptoms. A large perforation can contribute to feeling congestion, but there may be other issues going on that would better address your symptoms.

Thanks for your question regarding your crooked nose. I can certainly see how your nose tilts quite a bit to your right. This is something that can be improved with a septorhinoplasty surgery. It's very hard to get it perfectly straight but it should be possible to improve the positioning overall.

It's hard to say whether your bridge would return to it's original appearance after removing your bridge rib graft.
The main consideration is how well your skin would shrink wrap back down over the underlying bone. This varies depending on your skin thickness and age.
I suspect that if you don't like the large appearance, removing the graft would be a good way to lessen that issue.

This is commonly due to a deviation of the septum in that area.
